We are offering an exciting opportunity for a Care Partner to join our team at our Wallsend office.
About the role:
The Care Partner works in partnership with clients and the care team to codesign a plan of care based on the clients goals preferences and assessed needs whilst promoting functional independence wellness and lifestyles choices. The Care Partner will liaise with internal and external stakeholders to ensure positive client outcomes within resource/budget allocations.
