Position Summary:
As an integral member of the interprofessional team the Patient Care Coordinator (PCC) acts as a clinical resource for the interprofessional teams. The PCC will be reporting to the Program Services Manager.
Reactivation Care Centre (RCC) at Buttonwood is a 30 bed unit which aims to improve restoration to help patients maintain social cognitive and physical functioning by leveraging patientspecific goalbased service planning. Patients serviced by the RCC do not require acute care but need to engage in activities to improve their function and independence provided less active medical management and more therapy and activity. RCC also serves several health system purposes including:
- Reducing ALC pressures on acute care hospitals reducing the need for LongTerm Care Home placement and
- Decreasing the risk of readmission to an acute care hospital
Duties:
- Play a leadership role in providing direction ongoing development for nursing staff and other interprofessional team members in the clinical areas
- Perform financial/administrative responsibilities to ensure efficient and effective unit operation
- Contribute to staff development by promoting and ensuring a supportive/collaborative learning environment based on the principles of adult learning and practice standards for staff and students
- Perform human resources responsibilities in collaboration with the clinical program services manager
- Liaise with members of the multidisciplinary team to ensure high standards of quality and optimal management of patient care outcomes (including data collection/reporting) and ongoing continuous
Qualifications :
- At minimum a completion of BScN Baccalaureate in Nursing or Baccalaureate level education with Diploma in Nursing required.
- Current certificate of competency (licensure) from the College of Nurses of Ontario required.
- Basic Cardiac life Support (B.C.L.S.) program required
- Three (3) years nursing experience including experience as required by area of responsibility and experience working in a supervisory capacity.
- Demonstrates membership within a professional association and/or professional interest groups related to practice preferred
- Demonstrated strong leadership and motivational skills with ability to helping staff understand the relationship to their work and organizational goals
- Demonstrates ability to work in a team environment and collaborates with others in assisting with the delivery of care to patients
- Demonstrated leadership abilities
- Excellent skills in recognizing analyzing and resolving problems
- Excellent time management skills
- Excellent decision making and problem solving skills
- Clinical and patient teaching skills
- Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards

- Maintains and protects patient confidentiality in accordance with UHN Confidentiality policy requirements
