Job Description

Salary: $4800.00

Catholic Charities of Baltimore Senior Communities is currently seeking aService Coordinator who will educate and engage older adults in accessing information resources and services to support aging in place in an affordable apartment setting. This role will regularly assess resident and community needs and work to coordinate resources to better meet those needs to foster a safe healthy environment for older adults to age in place. The work schedule is Monday Friday 8:30am4:30pm.

Catholic Charities Senior Communities develops and operates affordable supportive communities for older adults with a resolve to nurture a spirit of purpose wellness and harmony among both our residents and colleagues. We offer 24 locations in Anne Arundel Baltimore Garrett County and Harford Counties and Baltimore City.

JOB DUTIES & RESPONSIBILITIES:

  • Builds oneonone longterm trusted relationships; assesses service needs in such areas as health care counseling nutrition transportation finance home management and legal services; determines eligibility for benefits and services; provides case management; and creates mutually beneficial relationships with organizations and services of the local community.
  • Develops relationships and acts as a liaison between residents and community agencies service providers and older adult advocates.
  • Responds to difficult situations including issues of selfneglect cognitiveimpairment chronic mental health concerns and crises.
  • Maintains accurate entry in American Association of Service Coordinators (AASC) online of all interactions and documents contact with residents their caregivers and providers including all followup daily.
  • Provides information in a variety of formats and methods including an accessible directory of resources and a community bulletin board.
  • Serves as the Volunteer Supervisor for the senior community nurtures resident and nonresident volunteerism and maintains Better Impact the agency volunteer management software.
  • Creates and maintains partnerships and mutually beneficial relationships with organizations and service providers in the local community and requests donated goods and services while maintaining all necessary paperwork required for the partnerships.
  • Plans and promotes wellness and educational events and programs for residents relative to aging in place safely in the affordable apartment setting.
  • Performs other duties as assigned.

EDUCATION & EXPERIENCE REQUIREMENTS:

  • Bachelors degree in human services or related field.
  • Three (3) years experience with older adults in the community or in a longterm care setting (group home skilled nursing facility or day care) or an equivalent combination of education and experience.

REQUIRED SKILLS & ABILITIES:

  • Ability to act with discretion tact and professionalism in all situations.
  • Shows reliability by being punctual; maintains consistent attendance.
  • Excellent communication and relational skills including active listening skills understanding and experience working with people of diverse backgrounds experiences and abilities.
  • Ability to utilize or willingness to learn computer systems and software necessary to perform position functions. (Basic computer web browsing online training and email usage skills.).
  • Ability to work with minimal direction or supervision.
  • Maintains a safe environment for residents and coworkers.

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:

  • Remaining in a stationary position often standing or sitting for prolonged periods.
  • Moving about accomplishing tasks or moving from one work site to another.
  • Medium work that includes moving objects up to 50 pounds.
  • Communicating with others to exchange information.
  • Repeating motions that may include the wrists hands and/or fingers.
  • Assessing the accuracy neatness and thoroughness of the work assigned.
  • Hearing and vision abilities within normal range (corrected) to drive safely and to observe and communicate with others.
