Make an Impact as a Family Care Coordinator!
The NewPath Child & Family Solutions Family Care Coordinator plans implements and documents mental health groups provides case management services and ensures continuity and family engagement between the program and home environments. The goal of this position is to assist and facilitate youth with meeting their treatment and/or education goals ensuring that their transition out of the Residential Program is successful.

What youll be doing:
Provide enhanced treatment continuity and family engagement for youth across the program and home environments.
Plan and conduct individual and family treatment interventions as indicated in the treatment plan.
Make contact with new clients and their families within 2448 hours of their admission.
Refer/link families with appropriate services in the community to meet their ongoing needs.
Attend treatment meetings home visits and health appointments to advocate and coordinate treatment.
Assist each youth in achieving goals and objectives.
Facilitate all aspects of daily group activities including planning implementation documentation of services and followup.
Provide enhanced service delivery through experiential or expressive modalities.
Submit lesson plans for group within designated timeframes.
Participate fully and positively as a member of the treatment team.
Contribute to transition planning and reintegration of youth.
Deliver culturally competent services to youth.

Qualifications:

Minimum qualifications include a High School diploma and 5 years experience with SED children.
Bachelors degree in a relevant field and at least 6 months experience working with SED children preferred.
Ability to work flexible hours including some evenings and weekends.
Initiative and ability to work independently.
A willingness and comfort level in providing communitybased services to a diverse population of clients with the ability to remain calm in a crisis situation.
Excellent communication and problemsolving skills.
Must be a minimum of 21 years of age have a valid drivers license and proof of current auto insurance with no more than 5 points on your MVR.
Must be able to pass a criminal background check.
