About the role and where you will be working:
We have a great opportunity for an enthusiastic Lifestyle Coordinator for Jeremiah Donovan House in Forrestfield. If you are passionate about working in partnership with older people supporting their wellbeing and maintaining their connection with community this could be the role for you.
As the Lifestyle Coordinator you will develop implement and monitor an activity program that ensures individual resident needs values spiritual and cultural preferences are taken into account. You will champion resident choices goals and independence. You will coordinate a team of Therapy and Lifestyle Assistant staff and volunteers working collaboratively with Occupational Therapists Physiotherapists and other health professionals to ensure provision of optimum level of personcentered service for all 65 residents.
This is a permanent fulltime position working reporting directly to the Facility Manager and an important part of the multidisciplinary team at Jeremiah Donovan House.

About you:
To be successful in this role you will have experience in lifestyle or allied health assistant services within residential aged care or a similar role and a strong commitment to person centered care. Most importantly youll have a genuine desire to make a positive difference in our residents lives.
