We are looking for Two Transitional Age Youth (TAY) Support Worker to join our Black Health and Social Services Hub Multidisciplinary team!
Reporting to the Program Manager the Transitional Age Youth (TAY) Support Worker bridges the gap between Child and Adolescent Mental Health Services (CAMHS) and Adult Mental Health Services (AMHS) by providing clinical interventions and supports education and advocacy to Black African and Caribbean youth experiencing complex mental health and/or substance use challenges.
The TAY Worker provides clients with a comprehensive care plan supporting both addictions and mental health needs using a stepped care approach. The TAY Worker conducts comprehensive assessments and provides clientcentered culturally affirming interventions. This position works closely with other professionals with expertise and linkages between mental health addictions and primary health care.
This role also seeks to address the systemic health inequities experienced by Black populations as described in the Ontario Health Black Health Plan by helping youth receive care for their health concerns while also connecting them to other social and community supports that can help improve their overall health and wellbeing.
Status: Permanent fulltime 37.5 hours per week ( evening hours and weekends may be required); Note this posting is for an active vacancy.
Salary: $61201..19 annually. (Candidate qualifications will be considered when determining the starting salary)
Location:This position is based out of Brampton.Although we operate within a hybrid workplace model staff are required to have onsite and/or community presence as per program and client needs.
What You Will Do:
- Provide mental health and addiction services and system navigation for transitional age youth (1624) and their support networks
- Facilitate coordinated access to community supports that is developmentally appropriate comprehensive and flexibly adapted to the unique needs of youth
- Conduct needs assessments develop and implement comprehensive care plans and ongoing monitoring until discharge
- Provide case management taskbased interventions and activities of daily living related to social determinants of health
- Assess and provide crisis support and plans make connection to crisis services as appropriate
- Refer and link clients to community resources that support recovery goals and address social determinants of health
- Facilitate groups related to the social/recreational needs of youth and life skills including development of new groups as needed
- Build on current and new partnerships to enhance coordination across the full continuum of community services resulting in positive outcomes including avoiding unnecessary Emergency Department visits

What You Need to Join Our Team:
- PostSecondary diploma or degree in Social Services or degree in relevant discipline (BSW MSW OT RN)
- Being a registered and / or regulated health professional is an asset
- Minimum 2 years of experience in transition or case management support
- Lived experience to support clients validate their experiences and support recovery
- Demonstrated cultural awareness and sensitivity in working with Black African and Caribbean (BAC) communities
- Strong understanding of historical and current health inequities antiBlack racism intersectionality and barriers impacting BAC communities
- Skilled in operating within an antioppressive antiracist traumainformed feminist and culturally sensitive framework
- Knowledge of local cultural programs and services for appropriate referrals
- Experience using evidenceinformed culturally adapted treatment modalities
- Strong interpersonal skills with proven ability to approach listen build trust and communicate across diverse cultures and intersectionality. Willingness to continually improve understanding around cultural competence.
- Openness to operate from an antiracist antioppressive 2SLGBTQ affirming and intersectional framework.
- Willingness to learn and apply practical understanding of different elements of diversity such as gender identity/expression race/cultural background religion sexual orientation disability immigration and precarious status family status etc.
- Fluency in a second language including ASL (American Sign Language) is considered an asset.
- Valid Canadian drivers license and access to an insured automobile in good repair.
- A satisfactory Vulnerable Sector Screening (Police Check) report.
