We are looking for Two Transitional Age Youth (TAY) Support Worker to join our Black Health and Social Services Hub Multidisciplinary team!
Reporting to the Program Manager the Transitional Age Youth (TAY) Support Worker bridges the gap between Child and Adolescent Mental Health Services (CAMHS) and Adult Mental Health Services (AMHS) by providing clinical interventions and supports education and advocacy to Black African and Caribbean youth experiencing complex mental health and/or substance use challenges.
The TAY Worker provides clients with a comprehensive care plan supporting both addictions and mental health needs using a stepped care approach. The TAY Worker conducts comprehensive assessments and provides clientcentered culturally affirming interventions. This position works closely with other professionals with expertise and linkages between mental health addictions and primary health care.
This role also seeks to address the systemic health inequities experienced by Black populations as described in the Ontario Health Black Health Plan by helping youth receive care for their health concerns while also connecting them to other social and community supports that can help improve their overall health and wellbeing.
Status: Permanent fulltime 37.5 hours per week ( evening hours and weekends may be required); Note this posting is for an active vacancy.
Salary: $61201..19 annually. (Candidate qualifications will be considered when determining the starting salary)
Location:This position is based out of Brampton.Although we operate within a hybrid workplace model staff are required to have onsite and/or community presence as per program and client needs.
