About us

Community Care Tasmania is a not for profit focusing on providing compassionate care and support enabling our clients to thrive at home with dignity. Our Launceston team are based in a brandnew state of the art building. The team provides state wide support to CHSP HCP DVA Community Champions clients and NDIS participants.

We are seeking an Onboarding Coordinator to join our Launceston team. This is a full time position and may require some travel within the state from time to time.

Summary:

The Onboarding Coordinator is responsible for managing the intake process for all prospective Participants seeking Aged Care and Disability services in a timely manner ensuring all opportunities to grow our Participant numbers from referrals are fully utilised. This includes assessing the needs of new Participants ensuring all information across all service portals is checked and managed daily ensuring compliance with relevant aged care and disability standards (legislation quality framework and funding guidelines) and supporting Participants and their families throughout the onboarding process. The position requires a strong understanding of aged care and/or NDIS care services excellent communication skills and a personcentred approach to support prospective and new Participants to Community Care TASMANIA. The Onboarding Coordinator is required to show initiative and work collaboratively with all staff across Participant Services to fulfil the position objectives whilst working within organisational policies and procedures.

Duties include but not limited to:

  • Participant Intake Assessment
  • Conduct initial assessment of new Participants including gathering relevant information about their health personal care needs and living circumstances.
  • Develop a care plan in collaboration with the Participant their family and healthcare and/or allied health professionals.
  • Ensure that all necessary documentation is completed and processed in compliance with relevant aged care and NDIS regulations and standards.
  • Identify any immediate needs or concerns and/or risks including safety mobility and medical requirements.
  • Act as the main point of contact for Participants Families and Allied Health providers throughout the intake and initial stages of service.
  • Support potential Participants carers/families to register with My Aged Care to access appropriate assessment and referral options to Community Care TASMANIA.
  • Keep Participants up to date with the status of their referral in accordance with organisational procedures.
  • Work with other team members to assign appropriate care staff such as support workers nurses and allied health professionals based on the Participants individual care plan.
  • Ensure timely service commencement including addressing any issues that arise promptly.
  • Ensure all scheduling of visits in place to set up for ongoing support management from relevant coordinator.
  • Liaise with the aged care NDIS and clinical teams where required to ensure a holistic approach to intake and onboarding.
  • Ensure that all Participants records are maintained accurately and confidentially throughout onboarding and service commencement
  • Stay up to date with relevant legislation guidelines and best practices in aged care and disability homebased services.
  • Ensure compliance with the Aged Care and NDIS Quality Standards.
  • Collect feedback from Participants and families through a structured feedback loop to ensure continuous improvement of intake and onboarding experience.
  • Manage declined referrals and intakes reporting to Manager Aged Care and NDIS (and to ELT when required).
  • Support timely and accurate reporting to funding bodies using Participant Management System Excel and other relevant databases or reporting tools.

About you: (Selection Criteria)

We are looking for an organised and enthusiastic individual who wants to make a difference and holds the following skills.

  • Tertiary qualifications in Management Business Allied Health Community Services or equivalent with subsequent relevant experience in a similar position.
  • Experience in a community based organisation providing aged care and NDIS services.
  • Has a good knowledge of other services in the specific region that the Participant can be linked to.
  • Knowledge of a range of support services in the state.
  • Demonstrated experience providing administrative support to a mobile workforce in a community or healthbased setting.
  • Previous experience supporting Participants through an intake role.
  • Demonstrated ability to work cooperatively as a member of a professional team.
  • Demonstrated ability to work autonomously with a high degree of initiative.
  • Ability to prioritise workload demands and cope calmly effectively and efficiently under pressure.
  • Proven capacity to maintain confidentiality and respect the privacy of Participants managers and other employees.
  • Proficient in the use of computer software including but not limited to word excel internet and email.
  • Police check (less than 3 months)
  • Current WWVP with NDIS endorsement
  • Drivers licence
