Job Description

Peer Support Specialist at Berea Primary Care Clinic
11AM 7PM Monday through Friday with some evenings and weekends.

PRIMARY FUNCTION

The Peer Support Specialist (PSS) supports the recovery needs of patients receiving Substance Use Disorder treatment. The PSS is an individual in stable recovery who is able to use their personal lived experience in recovery to guide and advocate for patients during the recovery process. The PSS serves as a connection between patients struggling with substance use disorder and recovery supports available to them. The PSS will engage individuals in all points of recovery to teach and model the skills necessary to support long term recovery. The role requires flexibility and selfdirection as well as the ability to problem solve to assist patient in meeting their recovery goals.

PRINCIPLE DUTIES

  1. Meet with patients during medical and/or behavioral health visits to assist in engaging patients in SUD treatment services. Work with care teams to:
    1. Develop care plans that meet the patients needs.
    2. Enhance patients motivation to engage with treatment services through the use of motivational enhancement strategies and sharing of personal recovery journey;
    3. Offer referrals and connect patients to alternative levels of care when needed; and/or
    4. Schedule patients for SUD services at White House Clinics.
  1. Maintain telephone contact with patients awaiting SUD treatment services at WHC.
  1. Work with patients to develop a PersonCentered Recovery Plan. Engage patients in developing recovery goals and a plan to develop skills strengths supports and resources to be successful.
  1. Remind patients of upcoming appointments pill counts random UDS etc. and help identify patient barriers. Work closely with all members of the care team to ensure patient compliance while promoting patients personal accountability. Follow up with patients who have missed appointments.
  1. Identify and share community recovery support meetings (i.e. AA/NA Celebrate Recovery etc) with patients. Attend community meetings to model positive behaviors for patients. Provide transportation for patients to meetings if needed. Encourage patients to remain active in community recovery programing.
  1. Provide 24 hour Telephone Support Recovery to patients at risk of relapse while receiving treatment at WHC including weekly contacts with patients and regular updates with care teams. Be available to respond to telephone calls from patients reaching out for recovery support during and after clinic hours.
  1. Serve as a liaison between the treatment team and the patient. Document all contacts in the patients EHR.
  1. Meet twice a month with assigned supervisor for required supervision according to state regulations. Supervisor will maintain a history of all supervision meetings.

JOB REQUIREMENTS

Minimum Education:High School Diploma or GED.
Minimum Work Experience:N/A
Required License:Kentucky Certified Peer Support Specialist. PSS is responsible for meeting all required supervision and continuing education requirements pursuant to 908 KAR 2:220. White House Clinics will provide supervision for PSS with an organizational LCSW.A valid drivers license and a reliable personal vehicle.
Qualifications
Must have been in stable recovery in the community for a period of at least 2 years. PSS must have a commitment to helping the Substance Use Disorder population and hold values consistent with those of the MAT services offered by WHC. Successful PSS will hold a commitment to and passion for helping individuals enter and maintain recovery. Successful applicants will be mature and well organized with pleasant personality have a professional and warm demeanor that facilitates the development of a collaborative relationship with patients providers and outside resources. Successful applicants will demonstrate strong problemsolving skills initiative and the ability to work independently. Ideal applicants approach the work in an energetic and creative manner. PSS will maintain healthy boundaries with patients and staff and adhere to ethical guidelines of the PSS role.

ACCOUNTABILITY

Accountable to Patient Support Services Service Line Manager. Will receive supervision from an organizational LCSW or the Director of Behavioral Health Services.

TYPICAL WORKING CONDITIONS

Work is performed in an office environment and involves frequent contact with staff and the public. Position may involve dealing with angry or upset people. Evening and/or weekend work is required. Work may be stressful at times. Work will also be performed in the community to assist individuals in recovery with access to community resources. PSS should expect to meet with individuals in community settings such as the Emergency Department and Jail in addition to the clinic.
WORK HOURS

37.5 hours per week. PSS should anticipate work to occur outside of normal business hours on a regular basis due to the nature of recovery coaching services. PSS will participate in an on call rotation which will require responding to telephone calls outside of clinic hours.
