Job Description

As a School Reintegration Coordinatorwith The Menta Group you will support students as they reintegrate from Menta to their home schools. The reintegration coordinator facilitates the successful transition between the therapeutic day school and the students home school and provides ongoing support through a variety of tasks (i.e. home school visits meetings with home school staff consultation with home school teams).you will provide direct educational and social services to students as well as work with parents educators and other professionals to create supportive academic and social environments for all students

DirectHire Salaried position with benefits and paid benefit time off.

HOURS 8am3:30pm MF following a School Calendar

Responsibilities

    • Facilitate the students successful reintegration between Special EducationServices programs and Chicago Public Schools and ensure all reintegration objectives are met.
    • Participate in regular meetings (schedule to be determined by building administrator) with building administrators in order to provide updates on current reintegration students and upcoming candidates.
    • Complete meeting notes after meetings with administrators (administrator to review) in identified Reintegration Coordinator Materials folder on the Google shared drive.
    • On a weekly basis complete logs documenting all reintegration activities for individual students and save in the Reintegration Coordinator Materials folderon the Google shared drive. Activities include but are not limited to calls to parents calls to home schools visits to home schools school meetings and IEP meetings.
    • Enter parent contacts and calls into the Reintegration Spreadsheet (each student has their own tab).
    • Provide specific updates and next steps to building administrators following all IEP meetings (also to be included in individual student logs on the Reintegration Spreadsheet).
    • Consult with the building principal regarding any issue or need prior to contacting the District Representative. If the issue cannot be resolved through the building administrator cc the building administrator on all communication with the district Representative.
    • Maintain all job related appointments on and off campus on your Google calendar and share the calendar with building administrators.
    • Complete individual student Making Action Plans (MAPs) Workplace Skills Assessments Living Skills Assessments and Technology Skills Assessments with all students (as appropriate for the students age/grade level) involved in any step of the reintegration process at least three weeks prior to their scheduled IEP meeting. Share documents with the students Case Manager and Clinician for IEP preparation. Document the date the MAP was completed on the Reintegration Spreadsheet.
    • Maintain a valid drivers license.
    • Supervise students in groups and individually monitoring behavior to ensure that it aligns with programmatic expectations.
    • Provide educational direction and support for students at times of confusion frustration & emotional upset.
    • Develop and update IEP goals and progress reports for student caseload.
    • Provide counseling or social work related service minutes as dictated on the IEP.
    • Respond in a timely manner to student requests for clinical services.
    • Monitor the collection of and analyze behavioral data.
    • Present information at staffings staff meetings and to clinical staff regarding academic behavioral and/or social progress.
    • Coordinate and communicate with administrative staff teachers and other related service staff as needed or IEP directed.
    • Have awareness of other students in the program (outside of their caseload) in
    • order to ensure consistent application of the therapeutic program.
    • Maintain confidentiality of students and student records.
    • Attend all staff meetings and inservice training as requested.

Qualifications

    • Minimum of an Associates Degree and ability to provide proof of the degree (A Credit Hour equivalent will not be considered. Degree must be issued.)
    • Educational/Job experience in Sociology Criminal Justice Psychology Human Services Social Work and Social Services is helpful
    • Strong organizational and interpersonal skills
    • Strong consultative and data analysis abilities
    • Openness and ability to work with students with marked socialemotional and other special needs
    • Case Management or Social Service experience is helpful
    • CERAP and risk assessment skills are helpful
    • Creative thinker who can develop a wide range of interventions meeting the needs of a diverse student population
    • Ability to operate tactfully under stress
    • Excellent communication skills
