Scheduled Weekly Hours:
40Work Shift:
Days (United States of America)Pathways Program: Provide care coordination for programeligible pregnant women to help ensure they have a medical home for prenatal and postpartum care practice appropriate pregnancy selfcare get connected to needed social services to remove barriers to care and learn basic maternal and baby care with the goal of achieving a full term healthy birth weight birth. Care coordinators educate clients on topics including safe sleep practices benefits of breastfeeding and dangers of tobacco use. Services are clinic and homebased.
Preferred: Certified Community Health worker (if not certified be willing to obtain within first year of hire). Required: High School diploma or GED valid Ohio Drivers License and reliable transportation. Should have good oral and written communication skills enjoy working with people be a selfstarter have knowledge of community resources and have good computer skills.
