This position may cover the tricounty area (Berkeley Charleston and Dorchester.

HOURS: 8:00am 5:00pm Monday Friday

Case Manager Downtown Charleston and James Island areas

Primary Function/General Purpose of Position

The Registered Nurses primary function is to plan provide supervise and coordinate care to patients with an array of injuries diseases and compromised health states. The Registered Nurse provides comprehensive skilled nursing services to qualified patients in their homes so that the stated goals and objectives of the agency can be achieved. The nurse works with minimum daily supervision or support while performing interdependent practice in the field.

Essential Job Functions

  • Requires operation of a vehicle for our organization.

  • Due to this job requirement the selected candidate/employee must possess a valid current drivers license.

  • Must be insurable under our organizations vehicle insurance plan which will be verified by reviewing licensure and driving record.

  • Provides direct handson patient care following patients plan of care and current standards of practice for Nursing.

  • Documents all care provided coordination of services orders goals etc. in the patients electronic medical record in an accurate and timely manner.

  • Supervises care provided by the LPN and Home Health Aides to ensure that treatment goals are being met.

  • Coordinates with other disciplines agencies/companies families etc. to ensure that the care needs of the patient are met.

Licensing/Certification

  • New grad RNs with a BON temporary work authorization must obtain Registered Nurse license within 90 days of hire.

  • All other RNs must be currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multistate license as a Registered Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC.

  • Must have a current American Heart Association BLS for Healthcare Provider Card.

  • Primary Source Verification (if applicable): Nursing SC Labor Licensing and Regulation (LLR): or compact RN state licensing board.

Education

  • Graduate of an accredited school of nursing.

Work Experience

  • One 1 year of professional nursing experience in an acute care setting is preferred.

  • Home health experience is desired.
