The University of Miami UHealth Department of Site Disease has a full time opportunity of for a Senior Patient Navigator (H) to work at UHealth SoLe Mia Medical Center in Aventura. The incumbent provides support and guidance throughout the patients care continuum by helping patients navigate through the maze of doctors offices clinics hospitals outpatient centers insurance and payment systems patientsupport organizations and other components of the healthcare system. The Senior Patient Navigator also acts as a resource for lowerlevel Patient Navigators (H) to ensure the timely delivery of quality care and patient satisfaction.
CORE JOB FUNCTIONS
Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.
Ensures patients have access to insurance and social service programs for which they are eligible.
Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.
Identifies and resolves real or perceived barriers to care.
Educates and counsels patients on issues related to their health and life situations.
Maintains current knowledge of community health welfare resources and any other programs available to patients.
Handle patient communications and contact healthcare facilities for medical records.
Address care gaps with the nurse navigator.
Prepares tracks and processes medical records and pathology specimens. Reviews and file medical records for completeness; use tracking systems for accurate documentation.
Provides administrative support to the department by answering phones address inquiries and direct calls or take messages Copy fax emailing and mailing records while ensuring confidentiality.
Adheres to University and unitlevel policies and procedures and safeguards University assets.

CORE QUALIFICATIONS
Education:
High School diploma or equivalent
Experience:
Minimum 4 years of relevant work experience
Knowledge Skills and Attitudes:

Ability to maintain effective interpersonal relationships.
Ability to understand and follow instructions.
Ability to process and handle confidential information with discretion.
