The Opportunity:

If you are a dynamic motivated individual who is driven to create healthier communities through excellent patient care Saint Lukes has a fantastic opportunity for a fulltime Cardiac Transplant Social Worker on our Transplant Team. Saint Lukes is a faithbased notforprofit aligned health system committed to providing the highest levels of excellence in compassionate health care and healthrelated services. For over 135 years weve been dedicated to enhancing the physical mental and spiritual health of the diverse communities we serve.

Shift: Full Time Monday through Friday day shift

Job Duties:

  • Evaluate patients psychosocial status and readiness for transplantation
  • Assist patients in understanding and navigating the transplant process
  • Facilitate access to community resources and support services
  • Advocate for patients needs within the healthcare system
  • Develop and implement psychosocial interventions to enhance patients quality of life and success of transplant
  • Participate in multidisciplinary transplant team meetings
  • Document patient interactions and maintain accurate records
  • Provide crisis intervention as needed
  • Assist with endoflife planning and decisionmaking processes
  • Communicates and collaborates regularly with multidisciplinary team
  • Assists in development of the discharge plan
  • Makes postacute referrals and follow through to completion including placements and transportation
  • Mitigates discharge delays through timely interventions.

Transplant Social Worker Qualifications & Skills

  • Experience working in a healthcare or medical setting
  • Strong communication and interpersonal skills
  • Ability to work effectively in a multidisciplinary team
  • Proficiency in crisis intervention and conflict resolution
  • Understanding of financial assistance programs available to patients
  • Competence in cultural sensitivity and inclusivity
  • Experience in patient education and advocacy
  • LMSW or equivalent clinical licensure
  • Masters degree in social work (MSW)
  • Minimum of 23 years of experience in a healthcare setting
  • Strong knowledge of psychosocial assessment and interventions
  • Excellent organizational and timemanagement skills
  • Ability to handle sensitive and confidential information
  • Empathy and compassion for patients and families
  • Proficiency in medical terminology and healthcare systems
  • Strong problemsolving and decisionmaking abilities
  • Commitment to ongoing professional development and education
