Job Description

Job Title: Medical Social Worker

Work Location: Morristown New Jersey

Rate: $56/hr. on W2.

Duration: 3 Months ( Extensions)

Department: Care Management Morristown
Job Type: Travel Contract
Schedule: 8:00 AM 4:00 PM (Weekend requirement)
Hours: 7.5 hours/day 37.5 hours/week
Category: Clinical Allied Travel

Job Responsibilities (Key Duties):

  • Assess the patients psychosocial condition in relation to medical needs; develop treatment goals to improve patient/family functioning.
  • Conduct interviews with patients families and significant others through case finding or referrals to assess psychosocial needs.
  • Participate in multidisciplinary rounds and provide input on psychosocial context.
  • Serve as a resource and advocate for patients and families including in discussions around Advanced Care Planning.
  • Provide psychosocial support during illness adjustment and life transitions.
  • Collaborate with healthcare teams on inpatient and post-discharge care planning.
  • Stay current on available community programs/services and inform relevant parties.
  • Maintain collegial relationships with community agencies and develop service networks.
  • Coordinate comprehensive discharge planning for services like rehab SNF LTAC dialysis hospice or housing support.
  • Document psychosocial assessments interventions and discharge plans per policy and regulatory standards.
  • Support patients in accessing social services (e.g. food stamps financial aid transitional housing).
  • Evaluate program outcomes and assist with planning and performance improvement.
  • Maintain confidentiality in compliance with hospital policies and professional standards.
  • May accompany patients to secure benefits entitlements or support services.

Required Skills & Knowledge:

  • Strong communication negotiation and conflict resolution abilities
  • Familiarity with current trends in clinical practice and healthcare informatics
  • Knowledge of state and federal regulations applicable to social work
  • Experience with clinical performance improvement or rapid cycle change initiatives

Qualifications:

  • Education: Masters in Social Work (MSW) from an accredited program
Experience: Prior medical hospital social work experience strongly preferred
