Summary
Transitional Assistance Coordinators are members of a team responsible for providing tailored high quality housing search assistance for individuals receiving MassAbility services and enrolled in the MassHealth ABI/MFP waiver programs The Moving Forward Plan Demonstration (MFP DEMO) program and/or the Housing Search Entity (HSE) program. They are responsible for assessing the housing needs and preferences of individuals assigned to their caseload assisting individuals in obtaining necessary core documentation assisting with housing searches and completing housing applications facilitating housing interviews and lease signing meetings when necessary and making household essential purchases that are permissible by an individuals TA plan. Transitional Assistance Coordinators are responsible for treating all participants with dignity and respect and for ensuring the human rights of all Individuals are met.
Essential Job Functions
- Conduct at minimum monthly visits with each assigned individual to complete tasks associated with housing benchmarks and/or goals
- Responsible for producing accurate legible and timely documentation for Individuals including but not limited to data sheets housing applications and other required written communications (TA Plans).
- Complete housing needs and preferences assessments with assigned individuals
- Complete housing applications for/ with individuals as needed
- Assist individuals in accessing community resources including making appointments at local Social Security Administration offices town halls Registry of Motor Vehicles etc.
- Drive to identified community locations for housing assistance including but not limited to longterm care facilities apartment buildings or other housing opportunities and government or state offices. Provide transportation for individuals on caseload including scheduling medical transportation if needed
- Assists Individuals in attaining their housing goals and provides individualized support when necessary
- Act as an advocate for the client to ensure that their opinions are heard regarding services provided.
- Act as a liaison between the individual and community resources
- Once community housing is secured support individuals in lease signing and purchasing household essentials and furniture.
- Ensures that all information regarding Individuals is maintained with the highest level of confidentiality based on the policies and procedures regarding protected health information.
- Attend and actively participate in mandatory staff meetings transition meetings and trainings as well as supervision meetings.
- Serves as a Mandated Reporter and is required to advise direct supervisor MRC and DPPC immediately if abuse and/or neglect of an Individual is suspected.
- Communicate effectively professionally and respectfully with Individuals coworkers management guardians and others while on company time.
- Serve as an active contributing and problemsolving member of the team. Provide support to coworkers by sharing knowledge about community resources as appropriate for the role of support staff.

Experience/Education
- Bachelors degree in social work psychology sociology human services or other social service area
- Valid drivers license
- General knowledge of local housing resources and the processes of obtaining core documentation
- At least one year of experience in a position assisting individuals in searching for and applying for housing opportunities
- Consideration will be given to candidates with an equivalent combination of education and related work experience
- Ability to work independently

Physical Demands
While performing the duties of this job the employee is regularly required to stand walk sit reach with hands and arms stoop kneel crouch use hands/fingers talk and hear.
The employee must frequently lift and/or move up to 50 pounds.
Specific vision abilities required by this job include close vision peripheral vision depth perception and ability to adjust focus.

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job
the employee is exposed to outside weather conditions. The noise level in the work environment may be moderate to loud. The job tasks will be performed mainly in the community and require driving.
