About the role

Assist NDIS participants in coordinating and implementing support services based on their NDIS plan using a personcentred approach. Build strong relationships with participants their families NDIS coordinators planners and service providers to ensure effective support and service delivery.

The successful candidate will have a strong knowledge of the NDIS and NDIS funding ability to read and interpret client plans manage budgets and work within a KPI driven environment.

This role is permanent part time role 4 days/30.4 hours per week) which can be flexibly based anywhere in NSW or ACT with hybrid work from home arrangements.

Key responsibilities

  • Strengthen and enhance our clients ability to coordinate supports and participate in the community
  • Regularly monitor our clients NDIS plan to ensure services are being delivered within their allocated budget
  • Ensure mainstream services meet their service obligations to clients (i.e. housing education justice health)
  • Provide the client and the NDIA with reports on outcomes and success indicators within the agreed reporting frequency

Skills & experience

  • Strong understanding of the NDIS including funding and client plans
  • Demonstrated experience in working with people with a disability and mental health issues
  • Time management skills and ability to work within a KPI driven environment
  • Excellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDIS
  • Strong written and computer skills for the creation of plans programs reports correspondence and other documents
  • Established time management problem solving and conflict management skill
  • Ability to think creatively and use initiative

Successful candidates will be required to clear probity checks including National Criminal History Record Check NDIS Workers Screening Check and Working with Children Check.
