Position Summary: The Patient Advocate Intern is accountable directly to the Lead Patient Navigator.


Salary The rate of pay for this position is $18.00 per hour and is determined by the candidates education qualifications and experience. Indeed provides its own estimated salary calculator and is not affiliated with COHs range.


Highlighted Duties and Responsibilities:

  • Works across departments and with outside entities including insurance companies and pharmacies to eliminate or reduce barriers to healthcare.
  • Coordinates and facilitates patient access to needed services and medications with a primary focus on medication refills and prior authorizations.
  • Assists with health centers prescription refill line including processing requests troubleshooting coverage issues and notifying patients when prescriptions are ready for pickup.
  • Works closely with medical providers to submit prior authorizations and discuss availability of alternative medications.
  • Provides timely and culturally competent patient advocate interventions based on needs identified in primary care appointments or ad hoc communications with patients or other members of the care team.
  • Documents all patient interactions and billable services in eCW or other systems per policies and protocols. Patient Advocate activities must be documented at a minimum the same day they are performed.
  • Facilitates patient empowerment and engagement by promoting educated independent patient choice on all aspects of care and service delivery.
  • Coordinates with Outreach and Enrollment staff to ensure that patients receive assistance with enrolling in health insurance when needed. Coordinates with Patient Navigator team to ensure patients get connected to other resources including durable medical equipment and food assistance.
  • Transitions work to new patient advocates on key job processes under the supervision of the Lead Patient Navigator.

Requirements
  • High School Diploma or Equivalent required. Some college with healthrelated coursework or equivalent experience preferred
  • Experience working in healthcare preferred.
  • Strong organizational skills and attention to detail required.
  • Ability to provide telephonic services over extended periods of time.
  • Commitment to work with underserved populations and ability to work in nonprofit environments.
  • Demonstrated ability to function effectively in a team.
  • Ability to work with computers including electronic health records
  • Ability to work full time hours on site during internship period..
  • Ability to work from other COH locations and travel between COH sites required.

  • In relation to remote work versus onsite expectations this position is classified as the following: Onsite: A majority or all of the position is required to work onsite. Please note that remote work designations are subject to change or fluctuate at any point in time and the supervisor may require in person learning for a specific amount of time after hire.

Community of Hope is a missiondriven innovative and rapidly growing nonprofit. For 45 years we have provided healthcare housing and community support services to make Washington DC more equitable. As a Federally Qualified Health Center we provide medical dental emotional wellness and care coordination services for the whole family at three locations in DC. Community of Hope also strongly emphasizes maternal and child health with midwifery practice and the only freestanding birth center in DC. In 2024 Community of Hope provided about 50000 medical visits 6300 dental visits and 17000 behavioral health visits for about 16000 patients. Community of Hope is also one of the largest providers of homelessness prevention and housing services for 1600 households1384 families and 220 individualsexperiencing homelessness in DC. Community support through Family Success Center our WIC nutrition centers and perinatal care coordination for pregnant people experiencing homelessness reach hundreds more families.
