Senior Home Health Physical Therapist Rehabilitation Specialist
Were seeking a dedicated Senior Home Health Physical Therapist to deliver exceptional rehabilitation services in patients homes. As a vital member of our patientcentered healthcare team youll provide comprehensive physical therapy services while enjoying the autonomy and flexibility of a mobile practice. Core Responsibilities:
- Conduct comprehensive initial evaluations and develop personalized treatment plans
- Deliver evidencebased therapeutic interventions including manual therapy therapeutic exercise and specialized modalities
- Implement advanced rehabilitation techniques including dry needling and kinesiology taping
- Document patient progress through electronic health records (EHR)
- Coordinate care plans with interdisciplinary healthcare teams
- Provide patient and caregiver education on exercise programs and injury prevention
- Ensure compliance with healthcare regulations and maintain quality standards
- Adapt treatment approaches to diverse home environments
Required Qualifications:
- Masters degree in Physical Therapy from an accredited program
- Current state Physical Therapy license
- Minimum 1 year of home health experience
- Valid drivers license and reliable transportation
- Current CPR certification
- Strong documentation and time management skills
- Excellence in patient communication and education
