Were seeking a dedicated Hospital Liaison Recovery Coordinator to provide a hospital based service London. You will work in close partnership with Accident & Emergency teams to develop and implement care plans for frequent hospital attenders.
Where: London Victoria Hospital satellites at St Marys Hospital and Chelsea & Westminster Hospital
Full Time Hours:37.5 per week
Full Time Salary: 27861..35 (based on full time working hours pro rata for part time)
Allowance: 4133.14 Inner London Weighting pro rata
Contract Type: Fixed term until 31st March 2026
Fulltime hours at Change Grow Live are 37.5 hours per week. For parttime roles the salary and payments will be pro rata based on contracted hours.
