Job Description
We are seeking a (Social Worker) for a 12-week travel/contract assignment in Scarborough ME 04074.
- Shift: 7:00 AM End: 3:30 PM
- Guaranteed Hours: 40 hours per week
- Weekly Pay Rate: $1400 - $1600
- Conduct assessments and evaluations for individuals in need of social services.
- Provide emotional support crisis intervention and counseling to patients and their families.
- Develop and implement care plans based on patient assessments.
- Advocate for patients in the healthcare system coordinating services and resources.
- Collaborate with healthcare providers including doctors nurses and other specialists to ensure holistic patient care.
- Document patient progress outcomes and interventions in compliance with healthcare regulations.
- Maintain confidentiality and adhere to ethical standards and practices.
- Provide information and referrals to community resources including housing financial assistance and counseling.
- Participate in case reviews team meetings and ongoing professional development.
- Licensure: Licensed Social Worker (LSW) in the state of Maine.
- Proven experience as a Social Worker ideally in healthcare or clinical settings.
- Strong knowledge of social services and community resources.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational skills and attention to detail.
