Permanent part time 22.5 hpw) Working Monday to Friday between 9am and 5pm pattern to be discussed
Pay 13.21 per hour 25838 per annum) pro rata and great benefits including Health Cash Plan

Typical day as a Lead Support Worker (known by us as Coordinator)
- Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.
- Mental Health support
- Helping customers with daily living activities such as making meals shopping budgeting maintaining their tenancy and moving onto independent accommodation if they are able.
- Carrying out risk assessments support planning goal setting and regular reviews.
- Cuppas chats and catch ups. Theres no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring
- Passion to support our customers to live their best life working collaboratively with an eye for detail.
- Experience of creating personcentred support plans and supporting colleagues in working to work to the plans.
- Experience of coordinating and assessing customer referrals.
- The ability to workon your own initiative remaincalm under pressure and havea resilient approach

Youll join Debbie Senior Client Service Manager Becki Client Service Managerand our Dream team were super passionate about what we do! Were really proud of support we provide to our mental health customers and the great work the team does to ensure our customers have positive outcomes and become ready for independent living. Were a supportive bunch and like to be there for each other as much as we do our customers.
Job details
- Working 22.5 hpw between 9am and 5pm Monday to Friday. Pattern to be discussed.
- Where were flexible with you we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
- Able to use technology for creating and updating support plans complete online learning and to collaborate with colleagues.
- Youll need an Enhanced DBS check done and we pay for that.
