Your RoleThe Health Services Coordinator is an integral member of Sertoma Star Services dedicated team working collaboratively to ensure that every aspect of health services delivery is managed seamlessly for individuals with intellectual and developmental disabilities living in their community. This Coordinator is a key player in overseeing medication administration ensuring compliance with established protocols and provides consultative guidance to individuals staff and healthcare practitioners regarding health care recommendations. The position also involves coordinating internal healthcare clinics managing the scheduling of lab work and facilitating the smooth transition to communitybased services. Through these responsibilities the coordinator supports a professional health services environment and the successful of our goals to transition to community based health services. The Health Services Advocate at Sertoma Star Services is instrumental in ensuring equitable healthcare access for individuals with developmental disabilities bridging gaps in care and fostering a personcentered approach to health and wellbeing. Responsibilities Monitor and review medical appointments for compliance with physician recommendations and provide consultative support to staff individuals and healthcare practitioners as requested. Collaborate with the pharmacy as needed to ensure prescription orders are filled correctly communicate any medication changes as needed manage the consenting process for medications requiring written consent. Schedule and coordinate internal healthcare clinics including vision dental podiatry and primary care provider (PCP) services while transitioning to community services per strategic goals. Schedule internal lab work with a focus on optimizing efficiency through a systematic scheduling process. Coordinate the nursing assessment and SAMA process to ensure timely and comprehensive evaluations and filing of the assessments upon completion. Conduct reauthorization classes monthly as needed for continued care. File all medical records and assessments in accordance with regulatory guidelines and organizational policies. Assist with medication checkin procedures to ensure accurate tracking and documentation. Administer medication as directed by the Registered Nurse Trainer (RNT) to ensure adherence to prescribed treatments. Notify the RNT of any issues related to medication management adverse outcomes or changes in health status. Manage the monthly medication destruction process and related paperwork. Liaison with families when appropriate and as directed. Ensure delivery and or coordination of all Health Services are following Rule 115 Rule 116 CQL and agency policy and procedures.
Qualifications Licensed Practical Nurse (LPN) with at least 3 years of experience. Previous experience in healthcare coordination advocacy or a related field preferred. Familiarity with QuickMAR or similar medication management systems is a plus. Strong knowledge of healthcare compliance regulations and best practices. Excellent organizational skills with attention to detail in documentation and recordkeeping. Strong professional communication and advocacy skills to support individuals and ensure highquality healthcare services. Proficiency in healthcare record management systems and computer applications. Ability to work independently and manage multiple priorities effectively. Ability to perform effectively and efficiently in a fastpaced highpressure work environment.
