Position Overview: Under the direct supervision of the Resource Navigation Program Supervisor the Resource Navigator is responsible for supporting Harris County Public Healths resource navigation and outreach activities to connect residents to federal/state/local benefit programs and internal and external programs/services. The position will primarily work out in the field and at various events and locations within Harris County.
Duties & Responsibilities:
Provides application assistance and followup (as appropriate) to Harris County residents for federal state and local benefit programs.
Facilitates connections to Harris County Public Health services and other community resources/services to address identified needs/concerns related to the social determinants of health when applicable.
Conducts community outreach and facilitates community engagement initiatives aimed at enhancing awareness of resource navigation services including canvassing local businesses/organizations and attending community events.
Schedules appointments for community members at various HCPH Clinics to facilitate access to healthcare and preventative services.
Builds and maintains relationships with stakeholders and organizations to create and sustain innovative partnerships to reach target populations for the resource navigation and outreach programs.
Accurately documents all client interactions services provided and followup actions in the designated record/system. Ensures that information is documented timely and in accordance with organizational policies and confidentiality guidelines.
At least one 1 year of experience in community outreach customer service or social services.
At least one 1 year of experience working with diverse populations including lowincome undeserved and culturally diverse communities.
Licensure/Certification:
Valid Texas Drivers License.
Knowledge Skills and Abilities:
Intermediate proficiency in the use of laptop tablet and cellphone along with Microsoft Office Suite.
Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment.
Strong communication and interpersonal skills with the ability to engage community members and collaborate with community partners.
Ability to maintain confidentiality when dealing with sensitive information.

General Information
Position Type and Typical Hours of Work:
This is a full time position 40 hours per week.
May be required to work outside of normal business hours (evenings and weekends) and in various settings including participants homes community outreach locations and office environments.
Travel within Harris County jurisdiction will be required for outreach efforts meeting with participants and training.
Work Environment:
This job operates in both field and office environment. This role routinely uses standard office equipment such as computers phones photocopiers filing cabinets and fax machines.
Physical Demands:
Frequently required to sit or stand bend and use manual dexterity. May occasionally lift and move up to 20 pounds.
