Job Description
We are seeking a dedicated and compassionate Social Worker to provide coverage across multiple healthcare facilities as needed. The Social Worker will perform comprehensive psychosocial assessments develop and implement care plans and provide intervention management and support for atrisk patients and their families. This role involves working closely with the multidisciplinary team to address social determinants of health discharge planning and aftercare needs.
Key Responsibilities
- Assess patient needs and develop care plans.
- Coordinate discharge planning and aftercare.
- Address social determinants of health.
- Provide emotional support and advocacy.
- Maintain documentation and perform other duties as needed
Qualifications- Education: Masters degree in Social Work (MSW) from an accredited institution.
- Licensure: LCSW or LMSW preferred based on state requirements.
- Experience: 3 years in a hospital and/or healthcare setting.
