Headquartered in Pennsylvania TLC operates across multiple states delivering K12 alternative education comprehensive mental and behavioral health services coaching counseling and traumainformed training. TLC also partners with school districts universities hospitals police departments and other nonprofit organizations providing consultation to secure federal and state grant funding.
Position Summary:
The implementation of Intercept offers a unique opportunity to become immersed in an evidencebased program model to serve youth & their families. The Intercept program serves a broad population of youth and families including those involved with multiple child serving systems and those at high risk of removal from their families. The program specializes in diverting youth from out of home placements by helping their families safely maintain youth in their home environment. This position is responsible for providing intensive homebased services to families.
Duties and Responsibilities:
Provide individualized family sessions to safely maintain youth in the home (managing a small caseload).
Provide treatment in multiple settings.
Provide afterhours support to families using a rotating oncall schedule.
Respond to crisis situations as directed by Supervisor.
Provide transportation to families as necessary.
Attend group meetings (supervision clinical consultation and individual development and trainings).
Maintain required documentation such as biweekly treatment plans weekly session notes and clinical assessments.
Serve as advocate for family in order to gather information specific to intervention development.
Adherence to all ethical and legal guidelines including confidentiality and mandated reporting.
Education & Experience:
Masters Degree Social Work Psychology or related field or
Bachelor Degree Social Services with one year of experience in counseling and/or case management.
Strong organizational skills and attention to detail.
Ability to maintain a flexible schedule. (working on around family availability.
Excellent written and verbal communication.
