The Case Manager I is responsible for providing staff support and direct client care at two different office locations Ondusky in New Haven CT and Wellington in Madison CT. The Ondusky Center is considered scattered site whereas the Wellington Program serves clients who reside in the building where we work. Responsibilities include delivering comprehensive case management services to clients from two different caseloads including intake assessments development and implementation of individual care plans and coordinating with treatment providers and community resources. Staff are expected to drive companyprovided vehicles to transport clients to appointments as required by business position is nonexempt and works under the supervision of program management.
Working within Adult Services the Case Manager 1 works to support clients recovery from mental illness substance use and housing instability.
