Flex Team - LPN PRN

Position Summary

On a flexible asneeded basis provides clinical services to all residents residing at Communities of Christian Living Communities (CLC). These services will include skilled nursing assisted living and all shifts.  Coordinates care and services based on resident needs and individual preferences as determined by ongoing assessment and evaluation.  Performs duties with educationbased expertise to provide a holistic and personcentered model of care for residents.

Flex team members are expected to fillin for a minimum of 2 shifts per month.  Most of these shifts are 12hour shifts.  This position is considered PRN status and employees may sign up for multiple locations dates and shifts.  With these expectations the Community Staffing Team will communicate closely with team members toward meeting their preferences and CLCs needs.

Essential Duties

• Attend appropriate Resident Care conferences; maintain Resident Care Plans ensuring accuracy and relevancy.

• Recognizes and identifies signs and symptoms and uses initiative and proper judgment in coordinating interventions.
• Communicates with other nursing staff physician pharmacist resident family member and others with need to know on residents status.
• Ensures protection of resident privacy and confidentiality.
• Must ensure documentation is done via electronic medical record platforms and is accurate. 
• Prepares administers and documents medications according to physician orders. Ensures that needed medications are available and expired medications are destroyed per protocol.
• Communicates any concerns incidents and progress of residents to all relevant parties.
• Documents assessment findings interventions and followup.
• General oversight and coordination of services during shift.  Will report any concerns to the Director of Nursing or appropriate supervisor.  
• Completes all required onboarding trainings and periodic training updates thereafter.
• Other duties as assigned.

Qualifications :

Basic Qualifications & Experience

• Colorado State Licensed Practical Nurse license in good standing.
• Ability to work well with different teams in different Communities.  
• 1year experience in a longterm care facility preferred and coordinating care in a clinical environment.
• Experience in caring for residents with dementia a plus.
• IV Certification helpful.
• Must have computer word processing and database experience.
• 1 year experience using electronic medical record platforms preferred.
• Must be able to read write and speak the English language.
• Must possess excellent customer service skills.

Working Conditions / Physical Requirements

• Moderate physical effort required by handling objects up to 50 pounds occasionally and/or up to 10 pounds frequently.
• Possible exposure to chemicals blood borne pathogens and other infectious diseases.
• Sits stands bends and moves intermittently during working hours.
• Is subject to frequent interruptions.
• Interacts with residents family members visitors frequently.
