The Centre for Addiction and Mental Health has partnered with the Regional Geriatric Program of Toronto (RGP)Psychogeriatric Resource Consultation Program (PRCP) in the Greater Toronto Area (GTA) to provide knowledgetopractice (KTP) services. These services are provided to individuals in longterm care homes and community service agencies that are caring for seniors with behavioural disorders associated with a dementia and other mental and neurological illnesses and vulnerable adults who may be less than 65 years of age but whose cognitive and functional abilities are compromised by early onset dementia developmental disability or acquired brain injury. The CH&E Coordinator/Psychogeriatric Resource Consultant (PRC) position liaises with appropriate inhouse services and local psychogeriatric outreach and Behavioural Supports Ontario (BSO) services. The Coordinator is part of a team of PRCs distributed across the region.
While based at CAMH the consultants work will be focused in longterm care homes and community service agencies in an assigned catchment area.
Core functions of the role includes:
- Assessment of learning practice development and implementation needs.
- Provide education training coaching implementation and other KTP services to organizations teams and individual caregivers.
- Provide training and staff support related to standardized curricula such as PIECES Gentle Persuasive Approach (GPA) BSO core curriculum and others.
- Provide guidance in the knowledge translation and implementation of recommended assessment and intervention plans.
- Provide guidance in establishing linkages with other services and healthcare providers.
- Participate in the continuous improvement of the PRC program services.
- Engage in collaborative KTP services with other members of the PRC team.
- Integrate KTP services with those more clinical services for the target populations as appropriate.
Reporting jointly to the CAMH Manager of the Geriatric Mental Health Outpatient Clinics and the Director of the RGP the candidate will support a healthy workplace that embraces diversity encourages teamwork and complies with all applicable and regulatory requirements.
The CH&E Coordinator/PRC will be required to provide services in accordance with the PRCPs KTP framework leading to practice and workplace improvements within the serviced organizations and enhance service integration. The CH&E Coordinator/PRC will also optimize opportunities to collaborate with other PRCs and participate in the programs accountability initiatives. The Consultant is responsible to adjust her/his work schedule to meet the needs of their assigned organizations.
- The successful candidate will have a Masters degree in Nursing Social Work Rehabilitation Sciences or Psychologyandthree years of related experience and eligible membership in their respective colleges.
- Experience in psychogeriatric services is preferred otherwise significant experience in geriatrics longterm or communitybased seniors care is essential. The candidate must have demonstrated competence with standard psychogeriatric assessment tools.
- Knowledge and experience in adult education and proficiency in the use of computing software such as Word PowerPoint Email and the Internet is essential.
- A valid drivers license and the ability to travel across the region and the GTA on a daily basis is required.
- Experience with standardized curricula such as PIECES. UFirst! Gentle Persuasive Approach BSO Foundations is an asset.
- The Candidate must possess knowledge/experience in the knowledge to practice process. Experience with facilitation of groups; adult education and training; development of work and communication plans is an asset.
- The candidate should be able to demonstrate critical thinking time management interpersonal communication and problem solving skills. Strong oral and written communication skills and the ability to manage a diverse task load are required.
- The successful candidate must be able to work independently and possess the ability for selfdirected learning but is also able to collaborate well with team members with excellent networking leadership relationship building interpersonal and listening skills to work effectively with a broad range of stakeholders and to facilitate the development of partnerships.
- Demonstrated understanding of the social determinants of health and the principles of health equity and how they apply in the context of evidence and knowledge exchange is required.
- Bilingualism (French/English) or the ability to speak in another language is also an asset.
