Job Summary
Utahs pediatric mental health care access program CALL UP (Consultation Access Link Line to Utah Psychiatry) under a grant awarded by the U.S. Department of Health and Human Services Health Resources and Services Administration HRSA Pediatric Mental Health Care Access Program PMHCAP is expanding to help more primary care providers identify diagnosis treat and refer children/adolescents/young adults for behavioral health conditions. The CALL UP Care Coordinator position in the Department of Psychiatry will be responsible for the continuum of care integration with the patient primary care and treatment protocols of psychiatric care. The Care Coordinator will serve as the liaison between primary care provider and the consulting psychiatrist. A critical component of the CALL UP program the Care Coordinator is expected to have extensive knowledge of regional resources and/or lived experience in order to successfully link primary care provider and families to the appropriate behavioral health services and supports based on their specific needs and insurance status. The Care Coordinator will also provide oversight of the workflow on the CALL UP consultation line.
Responsibilities
Essential Functions: Research and maintain accurate and up to date lists of local mental health resources and their availability including mental health providers family support services community resources peer support programs Respond to PCP requests with appropriate regional mental health resources within 24 hours including practice information such as participation with insurance and wait time for appointments When necessary work directly with families to help facilitate and coordinate referrals to needed services Provide followup calls to referred families within designated timeframes in order to ensure continuity of care Provide feedback as requested to PCPs on status of family engagement and services Support school personnel with referrals connection with PCP Establish med management and therapy and followup with EDs regarding patients PCP and therapy plans Record and log information from daily calls into CALL UP database Participate regularly in communitybased meetings to stay current on new and evolving resources Adhere to HIPAA privacy policies and hospital/health center regulations Work with the Program Director Medical Director to: Develop a Care Coordination plan that will provide navigation between primary care mental health therapy and school health care. Establish online/digital therapy curriculum for patients. Coordinate tele Crisis consults for rural and underserved areas of the state and warm handoff to crisis care Explore telehealth capabilities with clinics hospitals Tribal clinics etc.
Minimum Qualifications
Bachelors degree in a clinical area health care administration or related field or equivalency (one year of education can be substituted for two years of related work experience); two years of experience in case management activities with third party payers; and demonstrated organizational human relation and effective communication skills required. Licensure to practice in a clinical field preferred.
