Job Description
- Part Time Permanent Contract
- 16 hours per fortnight
Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute subacute aged care and community services.
The Cardiovascular CNC will provide high quality clientcentred care to cardiovascular clients in Specialist Clinics with a focus on providing timely follow up care for patients with Heart Failure and Transient Ischaemic Attack.
This role requires an advanced scope of nursing practice in the assessment management and collaborative care of cardiovascular conditions and forms an important part of the multidisciplinary team.
