Job Description
An exciting opportunity exists as a Client Navigator in our Community Behavioral Health Center! We are looking for a compassionate and professional team member to guide clients through the Same Day Access behavioral healthcare appointment process and continued followup.
Responsibilities include:
Manage the clients visit to Aspire from the intake process through the visit with the clinician and through the care process
Respecting the clients choice explain and collect informed consent for treatment and participating in the grant funded activities and ensure that clients are fully informed of their legal rights
Collect and accurately document intake assessments prior to the clinician visit
Manage several clients each day at different points in their initial visits and treatment plans
Support clients nonclinical needs with deep understanding of both Aspire and community resources to remove barriers to the clients recovery including addressing social determinants of health
Participation in Daily Care Huddle to help determine best course of action for each client
Takes vital signs and weight of client and record in electronic health record

Aspire Health Alliance has been designated by the state to operate a Community Behavioral Health Center (CBHC). Aspires CBHC serves MassHealth members from the South Shore specifically Milton Quincy Braintree Randolph Weymouth Hingham Cohasset Norwell Hull and Scituate.
Schedules:
Full Time: Monday through Friday 9:30 am 5:30 pm or 10am 6pm

Required Skills
Bachelors degree Required in a healthcarerelated field is preferred
Knowledge of the health care system and experience or ability to work with busy clinicians to help manage a successful client visit
Prior experience in researching and knowledge of community services available for clients
Prior experience in taking vitals preferred or willing to be trained.
Community Health Worker (CHW) Certification preferred
