Qualifications:
- Education:Diploma/Certificate from an accredited LPN program.
- Licensure:Active New York State LPN License; Basic Life Support (BLS) Certification.
- Experience:Prior experience in rehabilitation longterm care or similar settings preferred.
- Technical Skills:Proficiency in vital signs monitoring medication administration and EHR documentation.
- Soft Skills:Strong communication physical stamina empathy and attention to detail.
Key Responsibilities:
- Provide direct patient care including administering medications and monitoring vital signs.
- Assist residents with activities of daily living (ADLs): bathing dressing feeding and mobility.
- Report changes in patient conditions to RNs and physicians promptly.
- Maintain accurate and timely documentation of care plans and patient progress.
- Participate in care planning meetings and family conferences.
- Ensure compliance with facility safety and hygiene protocols.
