About the Role
An attractive position is available for an experienced allied health or nursing professional to join our Transition Care team working as a case manager based at St Georges Health Service in Kew.
The Transition Care case manager assumes a key role in the coordination and case management of clients within the Transition Care Program and works as part of a multidisciplinary team.
Duties
Direct provision of high quality and evidencebased patient centered care.
Undertake holistic comprehensive assessments in collaboration with the client using case management principles and practice frameworks including strengths based approaches.
Collaborative care planning with clients carers and/or families in the context of their health functional psychosocial cultural environmental and economic needs.
Facilitation of client generated goals that are realistic time limited and based on addressing health and wellbeing needs
The classification range from this position is:
Allied Health Grade 2 Year 14 $88467 $101691 per annum pro rata) or
Registered Nurse Grade 4A Year 12 Community Health (YW7 YW8 $per annum pro rata)
Your Contribution
Tertiary Allied Health or Registered Nurse qualification.
Minimum of 5 years experience in a healthcare setting
Demonstrated ability to work autonomously and as part of an interdisciplinary team
Demonstrated ability to effectively work with relevant service providers to achieve best outcomes for clients
Highly developed interpersonal skills that incorporate the qualities of excellent communication negotiation organisation planning and problem solving

About Transition Care Program
Transition Care services target older people at the conclusion of a hospital admission who require more time and support to complete their restorative process optimise their functional capacity and finalise and access their longer term care arrangements.
The team includes case managers physiotherapists occupational therapists podiatrists dietician speech pathology allied health assistants and administrative support.
