Job Description
Clinical Case Manager
- Participates in/conducts developmental evaluations/screenings consultationsdirect services and family assessments in collaboration with the multidisciplinaryteam when appropriate for the purpose of completing intake determiningeligibility monitoring progress updating IFSPs and planning/providingintervention.
- Works with IFSP team to ensure State and Federal timelines are met with regardto timely service delivery transition IFSP development/updates and LEA/SEAnotification.
- Recommends coordinates and makes referrals to community services to meetidentified outcomes on Individualized Family Support Plans.
- Maintains current and accurate client records including program plansintervention notes case notes and billing information timely and accurately forpayment.
- Participates in local and statewide meetings and trainings to ensure best practiceand compliance; Completes trainings/modules as required by the program andorganization.
$45500 to $57000 annually depending on education and experience.
Masters degree in an appropriate area; or a bachelors degree in an appropriate area and two years of relevant experience.
- Bachelors degree in Early Childhood or Early Childhood Special Education Social Work Psychology or Sociology.
- Two years of appropriate experience with children birth to five with special needs.
This is a timelimited position. Time limited positions are based on circumstances such as funding sources which control the length of time for which the position is available.
