Position Summary:
Chemik Hospice Care LLC is seeking a dedicated and compassionate Assistant Director of Nursing (ADON) to support the clinical leadership of our hospice team. The ADON will assist in overseeing daytoday clinical operations ensuring highquality patient care and supporting compliance with all state and federal regulations. This role is ideal for a motivated RN with strong leadership skills and a heart for hospice care.
Key Responsibilities:
- Assist the Director of Nursing in managing and supervising the clinical team including RNs LVNs CNAs and other field staff
- Support in planning directing and evaluating patient care services to ensure quality and compliance
- Conduct supervisory visits chart audits and monitor documentation for accuracy and timeliness
- Help coordinate patient admissions recertifications and discharges in collaboration with the IDG team
- Participate in training mentoring and evaluating nursing staff performance
- Ensure adherence to Medicare Conditions of Participation and state hospice regulations
- Collaborate with physicians families and referral sources to ensure smooth communication and transitions of care
- Participate in oncall rotation as needed
- Assist with preparing for state and federal surveys and audits
- Promote a positive teamoriented culture focused on compassionate endoflife care
Qualifications:
- Current and valid Registered Nurse (RN) license in the state of Texas
- Minimum of 2 years of nursing experience with at least 1 year in hospice or palliative care
- Prior supervisory or leadership experience preferred
- Knowledge of hospice philosophy and regulatory requirements
- Excellent communication leadership and organizational skills
- Proficiency in EMR systems (e.g. Alora WellSky Kinnser) and Microsoft Office
- Ability to handle multiple priorities in a fastpaced environment
- Current CPR certification
- Valid drivers license and reliable transportation
