Have you previously supported a loved one or client as a Trained Dialysis Care Partner but theyve since passed on or no longer require care

Whether youre a former family caregiver or a professional with dialysis training your experience is invaluable.
What Youll Do:
- Provide support during and after dialysis treatments ensuring safety comfort and emotional reassurance
- Assist with mobility daily living tasks and creating a clean recoveryfriendly environment
- Monitor for posttreatment symptoms such as fatigue or dizziness and communicate concerns promptly
- Support patient understanding of home dialysis protocols diet hydration and treatment compliance
- Collaborate with family members and facility staff to ensure continuity of care
- Support transitional care for those newly discharged or beginning dialysis at home
Who You Are:
- Youve received dialysis training (e.g. through DaVita Fresenius or a home hemodialysis (HHD) program)
- Youve worked as a care partner whether professionally or as a dedicated family caregiver
- Youre compassionate detailoriented and confident assisting someone with complex care routines
- Youre comfortable using a mobile app (Herewith) to accept shifts manage your availability and communicate with clients
- CPR/First Aid certification is preferred; CNA/PCT/HHA credentials are a plus but not required
