What Youll Do:
Under direct supervision of the Director of Access Services the transition coordinator is responsible for removing barrier providing support to the following programs: MST IIH PRTF (scanning only) Day Treatment (scanning only) as needed and ensuring clients have access to services within a reasonable timeframe.

Qualifications
Education and Experience
- Bachelors Degree required
- Must be QMHP as verified by AYN
- Behavioral health front desk medical office experience preferred.
- Proficiency in MS Office applications
- Valid Drivers License
