Case Manager

Intensive Youth and Family Worker supports vulnerable youth who need support to improve their resilience skills relationships connections to resources and readiness for independence. In this role you will collaborate closely with participants MCFD and family members or natural supports to help youth identify and accomplish their goals. We meet youth where they are at and provide oneonone or groupbased supports in office and in the community.

Key Duties and Responsibilities

  • Gathers information relevant to the youths problems needs and risks by interviewing observing behavior meeting with caregivers and service providers and using an assessment tool. Review the information gathered to identify strengths problems needs and risks.
  • Develops and implement shortterm issuespecific intervention plans within the program scope.
  • Plans prepares and conducts group or individual sessions using techniques such as active listening conflict resolution and problemsolving to resolve the identified problems needs and risks.
  • Assesses effectiveness of strategies used and checks in on youths progress toward goals shifting approach where necessary to best meet the needs of youth.
  • Maintains related records and statistics and provides reports to the supervisor as required
  • Liaise with other professionals and youths natural supports to promote the interests of youth and their families.
  • Perform other duties as required.

Qualifications

  • A diploma certificate and/or relevant experience working with children in recreation education or social services.
  • First Aid Certificate.
  • Class 5 drivers license and a drivers abstract

Hours: 27 hours per week flexible schedule Monday to Friday between 9:00 am5:00 pm

Rate of Pay: $24.13/hour
