DescriptionPRIMARY PURPOSE
Under the direction of a supervisor oversees the primary rehabilitation needs of the patient in Home Health Virtual Health Program. Assesses the patient and caregivers; establishes the care plan; teaches the patient; evaluates clinical outcomes; and assists with program development.
Major Responsibilities
- Provides video consultation/triage to patient family and significant others according to the Home Health Virtual Program plan of care.
- Provide ongoing care via video according to the established plan of care. Determines priorities of patient care based on essential patient needs and available resources.
- Recognizes changes in the patients status need for care and intervenes appropriately.
- Coordinates patient interventions such as physician contacts consultation with patients home health care team facilitation of appropriate visits strings changes in physician orders and care plan goals in cooperation with the patients physician as appropriate.
- Assesses patient family and/or caregivers training needs and provides teaching/counseling video consultation/triage.
- Collects and maintains clinical and patient records according to agency state and federal guidelines. Assumes primary responsibility for video documentation data review quality and appropriate followup of patient information.
- Assists in completion of reports and statistics related to key clinical performance measures.
- Ensures quality of care through development implementation and evaluation of professional standards.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her agespecific needs and to provide the care needed as described in the departments policies and procedures. MINIMUM JOB REQUIREMENTS
Education
Doctorate degree in Physical Therapy from an accredited school or education requirements commensurate with the date of graduation
Maintains continuing education requirements as mandated by applicable state professional standards
Certification / Registration / License
Physical Therapy License in the State of North Carolina and South Carolina (Compact license between North Carolina and South Carolina acceptable) must be licensed in the states upon which you work. Basic Life Support (BLS) per Atrium Health Life Support Policy
Work Experience
Requires 5 years of experience in combination of rehab outpatient and home care settings
Home Care experience preferred
Knowledge / Skills / Abilities
- Must possess strong communication skills be detail oriented and have the ability to coordinate and manage complex care for patients over video
- Must be motivated and selfdirected and possess above average interpersonal skills
- Demonstrated ability to work well with management physicians and other professional in a direct and positive manner.
- Demonstrated ability to work in a fastpaced environment and under time constraints.
- Advanced computer skills using Microsoft Office applications and overall proficiency and comfort level with computer systems.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
Generally exposed to a normal office environment.
Operates equipment essential to performing the duties of the job.
May require sitting/standing for long periods of time.
