Reporting to the Health Programs and Services Manager the Community Health Navigator is responsible for participating in the delivery of health programs to TH citizens that promote health education awareness prevention and healthy lifestyles in order to protect prevent and reduce the incidence of cancer and chronic conditions communicable diseases injuries disabilities and premature passings. As a member of the Wellness team and to ensure that citizens understand their conditions and treatment receive appropriate care assistance and supports the Community Health Navigator serves as an advocate for citizens and liaises between patients families Yukon Government health medical professionals and staff the federal NonInsured Health Benefits Program and additional partnerships when required. The Community Health Navigator meets these objectives through community health surveillance effective health programs and initiatives by using a collaborative case management approach and by ensuring that TH culture and traditions are included in health service delivery.
An Eligibility List may be established from this competition the duration of the eligibility list may be up to 12 months. The eligibility list may be used to fill future permanent fulltime permanent parttime temporary fulltime temporary parttime term or auxiliary oncall vacancies within the same department and classification based on the organizational needs by going the next highest ranked candidate until the eligibility list expires.

Essential Qualifications:
- PostSecondary Certificate / Diploma in Public Health Nursing Nutrition Social Work or a related field. An equivalent combination of education training skills and experience may be considered.
- Experience in communitybased health social or wellness programs.
- Knowledge of or willing to learnTrondk Hwchin government structure culture traditions history demographics family structures and socioeconomic dynamics challenges and aspirations.
- Plan organize implementdeliver and evaluate health related programs.
- Coordinate programs & obtain participation from citizens and other agencies.
- Computer proficiency.
- Provide advice and guidance on health care services to the community / staff.
- Ability to develop andmaintain positive relationships with the community citizens / families coworkers and other departments and outside agencies.
Conditions of Employment:
- Criminal Record Check with Vulnerable Sector Screening.
- Class 5 Drivers License with Clear Drivers Abstract.
- Standard First Aid certification or willingness to attain.
- Foodsafe Level 1 certification or willingness to attain.
- TH101 Cultural Awareness training.
