The Pueblo Regional Center (PRC) is one of three stateoperated regional centers providing quality specialized services and supports to individuals with developmental disabilities and mental health diagnoses. PRC manages 11 group homes throughout the Pueblo West community licensed to serve 88 individuals. PRC provides 24/7 care and treatment in homelike settings while offering habilitative services and supports in the areas of residential day
programming therapies and health and nutrition.

PRC is a 24/7 facility; therefore person in this position must be available to work any shift any day and any area to meet agency needs.


Description of Job:
This position is essential in the coordination of Community Integration and Habilitation Support Services (CIHS) for disabled individuals living at the PRC. This position manages a section (multiple workrooms or sites) of day program services. Responsibilities are the writing and implementation of individual plans for Community Integration and Habilitation Support Services (CIHS) services working with the coordinator to ensure PRC clients arrive make transportation connections and are placed in appropriate activities and employment sites with necessary support for the CIHS and employment navigator communicating with necessary team members to support medical/off campus appointments and helping to ensure staff coverage for all work and personal appointments are met. This position engages in communications with medical staff residential management case management and other team members in order to ensure proper management of people and resources throughout the CIHS day. This position is responsible for scheduling the calendar for activities for CIHS and supportive employment services.

Coordination of CIHS Services:
Develops and coordinates appropriate CIHS services for assigned group of clients including procedures and operating systems that govern the delivery of services in accordance with state federal and agency regulations. Participates in time studies required by the US Department of Labor according to established standards as needed. Ensures assigned areas have appropriate supplies and materials to meet their needs according to the Individual Support Plan (ISP). Focus of activities include program development program implementation supervision and leadership vocational assessments and environmental considerations development of group/work schedules activity plans and workroom organization/structure. Collaborates with professional discipline staff to ensure integration of all services within CIHS. Represents CIHS at IDT meetings management meetings and psychiatry clinics. Participates in team meetings with other program coordinators to ensure a smooth flow of comprehensive services to all PRC clients. Ensures all paperwork and documentation associated with agency and outside standards are met. Gathers assembles and analyzes facts to devise solutions to problems related to CIHS for the CIHS services director. Serves as backup to Community Integration Coordinator when gone. Coordinates CIHS scheduling and submits and coordinates calendar and communication to home staff.

Supervision:
Exercise supervision over the CIHS employees (apx 10 in the area of hiring performance planning performance evaluation leave approval and progressive discipline. Implements the overall operations of day program and implements regulatory compliance in conjunction with: DPA regulations; Personnel Rules; State Board of Nursing Practice Acts; Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICFIID); Health Facilities and Emergency Medical Services Division (HFEMSD) of Department of Public Health (DPH); Centers for Medicare and Medicaid (CMS); CDHS Policies and Procedures; and other state and federal mandates. Management of subordinate caseloads to ensure all needs are met. Monitors trains and observes direct reports to ensure compliance with all regulations and documents accordingly.

Medication Administration:
Administers physician ordered medications and medical treatments in proper dosages at appropriate times to each individual. Records a wide range of information: medication given (includes time and amount); residents responses to medications and treatments; intake output seizures menses; and all information relevant to all aspects of the clients medical status.

Direct Care Services:
Performs other duties as assigned.

Other Duties:
Implements and trains in the following areas: physically lift transport and/or position residents by using approved lifting techniques. Employ current physical interventions methods in addressing a behaviorally troubled resident. Documents outcomes of treatments and program implementation timely and accurately. Monitors and addresses deficits in progress maintenance and regression according to ISP. Safely operate a motor vehicle when transporting residents around Pueblo Regional Center (PRC) sites or to community locations. Ensures safe transportation of clients to work area program area medical appointments court hearings and community activities as well as taking an active part in vocational training. Assists residents with personal shopping; assists in ordering/purchasing supplies for the work site; follows Agency/State rules and policies and procedures; assures legal compliance with all purchases; and maintains agency mission. Arrange for residents to attend medical and outside doctor appointments order medications and treatments thus ensuring adequate delivery of medical services. Participates in the Interdisciplinary Team process discussing residents ISPs (through goal and objective training Safety Care training direct contact and monitoring). Formulation of accurate information for assessments and formal reports for inclusion in the team process and individual program plans (ISP).

Minimum Qualifications Substitutions Conditions of Employment & Appeal Rights

Minimum Qualifications:
High school diploma or General Education Diploma (GED).

AND

Three 3 years of experience in an occupational field related to the work assigned to the position with one of these years experience in a supervisory role.

Substitutions:
Additional appropriate education will substitute for the required experience on a yearforyear basis.

  • Shift Work PRC is a 24/7 facility; therefore person in this position must be available to work any shift any day and/or any area to meet agency needs.
  • Travel Position must travel to remote work sites and must utilize a State vehicle if available. If not must utilize personal vehicle and submit for mileage reimbursement.
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must 1 disclose that information on the application 2 provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position 3 provide the employee number from the applicants prior State employment. Absent extraordinary circumstances prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
