Case Manager Recover Support Specialist

$1000 Sign on Bonus

The Association for Individual Development (AID) is a nonprofit organization whose mission is to empower people with physical developmental intellectual mental health challenges; those who have suffered a trauma; and those at risk to enjoy lives of dignity and purpose. We are looking for aCase Manager Recover Support Specialist Crisis Care Counselor Mobile RSS who demonstrates this mission and wants to work for an organization that makes a difference in the community.

Schedule: 6:00 pm 6:00 am Monday Tuesday Saturday. As well as every other Friday.

What you will be doing

The Crisis Care System is a continuum of crisis response services comprised of a 24/7 Crisis Line and Mobile Crisis Response team. This position creates the opportunity for a person with lived experience who is in recovery from mental health diagnosis or cooccurring substance use disorder. A Crisis Care CounselorMobile Recovery Support Specialist is responsible for the following:

  • Utilize lived experience with mental health diagnosis to support clients with 1 decreasing mental health dysfunction 2 increase community functioning and 3 assist clients in achieving recovery goals.
  • Providing crisis intervention deescalation emotional support and suicide risk assessment to callers in crisis via phone through AIDs 24/7 crisis line.
  • Manage and assess incoming Mobile Crisis referrals from 988 local police departments and other community services via phone through AIDs 24/7 crisis line.
  • Provide in person Mobile Crisis Response with a Crisis Counselor anywhere in Kendall and Southern Kane County when a person is experiencing a mental health crisis that cannot be deescalated over the phone.
  • Collaborate with a team of service providers within AID and throughout the social service community to ensure that all clients needs are being met to promote and maintain community stabilization.
  • Maintain documentation required for crisis intervention services provided on the phone and in the community
  • Attend to scheduled work hours and be flexible to meet client and program needs as assigned by Program Manager or Director.
  • Provide effective individual crisis counseling and crisis intervention assessments.
  • Responsible for providing independent program coverage when scheduled or in emergency situations.
  • Complete clinical documentation appropriately to meet the requirements of program funders and applicable administrative rules.
  • Provide effective telephone counseling service through active and empathetic listening and motivational interviewing.
  • Complete required documentation on a timely basis
  • Must meet monthly minimum predetermined productivity standards. When Applicable
  • Complete all necessary assessments including the crisis intervention and suicide assessment and required documentation including billing notes in accordance with Rule 132 standards
  • Develops and implements crisis safety plans and resource follow up in collaboration with the client other providers and other parties as identified by the client following the standards of Rule 132 payers and accreditation bodies
  • Provide case management follow up for clients to support linkage to services
  • Maintain records of individuals served on the MCRT in accordance to Rule 132 payers and accreditation bodies. Assist MCRT Lead in conducting regular quality assurance of MCRT records.
  • Maintain open and effective communication with agency and provider staff family and others in response to clients identified needs and client assistance/service needs.
  • Navigate Cx360 have a working knowledge of opening clients to programming and ensure client profile is completed
  • Navigate crisis line documentation service working knowledge of submitting documentation finding frequent callers and submitting sunshine documentation
  • Recognize emergency situations and take appropriate action. Contact Manager and Director per procedure. Complete necessary paperwork correctly (incident reports petitions case notes)
  • Participate in training and development opportunities as assigned.
  • Promote awareness of mental health both internally and externally.
  • Fulfill other functions on the team as needed.
  • Adhere to all Program 590 guidelines and expectations as outlined by the 590 Mobile Crisis Response Grant and IDPH Department of Mental Health
  • Fulfill other functions on the team as needed.

What will you bring to the table

Education: High School Diploma or equivalent. Bachelors Degree in a Human Service field preferred. BilingualSpanish preferred

License/certifications: CRSS (Certified Recovery Support Specialist) certification preferred or certification requirements should be met within one year of hire and certification obtained within 18 months (about 1 and a half years) and maintained throughout employment.

Experience: Some mental health experience preferred individual must be in recovery from mental health illness or cooccurring mental illness and substance use disorder

What are the other requirements

  • Manage schedule in a manner that allows timely completion of all duties and be available to program after hours.
  • Be on time to scheduled shifts and trainings clock in and out per agency policy.
  • Maintain agency payroll system to ensure all hours are reflected accurately at all times add punch requests as necessary.
  • Request scheduled and unscheduled PTO utilizing the agency payroll system per agency policy. All PTO requests must also be sent directly to the Manager via email for approval.
  • Follow program and agency procedures for notifying management when unable to attend a scheduled shift/ training

Ensure the coordination of services in the community to meet client needs (financial legal advocacy medical etc.

  • Provide transportation to clients in personal or agency vehicles when necessary.
  • Acquire and maintain certifications in First Aid Adult CPR NonViolent Crisis Intervention (CPI) OIG Rule 50 Infection Control and Aetna Compliance and Fraud trainings as well as any other trainings assigned by Manager.
  • Attend and participate in agency and program committees events and fundraisers as assigned.
  • Maintain a positive and professional working relationship with coworkers agencies and other stakeholders.
  • Perform all other job duties as assigned and in accordance with the Association policies and procedures.
  • Promotes client wellness through coaching counseling consultation motivational enhancement and information and referral services to clients as needed.
  • Communicates with service delivery partners which may include Physicians Nurse Practitioners Counselors Pharmacists Employers Community Supports and other Health Professionals as needed to facilitate the achievement of clients wellness objectives.
  • Promote awareness and respect for the diversity of our clients employees families and other stakeholders.
  • Maintain a team approach.
  • Seek out both inter agency trainings and outside entities to increase professional knowledge and enhance clinical skills
  • Provide consultation to other AID staff to assist in managing crisis situations for agency clients.
  • Manage personal wellbeing to ensure adequate performance and professional judgment at work
