As our Lead Reablement Worker youll be at the heart of supporting our customers aged 55 to stay independent in their own homes. Youll lead our amazing team of Reablement Workers helping them deliver personcentred care that makes a real difference. From supporting people with daily living to coordinating care plans and working with health professionals youll be key to helping our customers live well and stay connected to their community.
Based from our fantastic new building on Scalby Road we are very close to Scarborough town centre on public transport or by car (you can almost smell the fish and chips and were sure that well be popping out often to grab some for our lunch!.
- Supporting customers with varying needs including dementia and frailty to regain or maintain independence in their daily activities.
- Developing and reviewing personalised care plans that focus on reablement safety and the customers aspirations.
- Providing guidance and leadership to Reablement Workers ensuring high quality person centred care is delivered.
- Managing more complex personal care needs including stoma and catheter care while promoting customer choice and dignity.
- Working closely with external professionals like health and social care teams to make sure customers get the best support possible.
- Using creative solutions including assistive technology to empower customers to live independently for as long as possible.
- Experience in a similar role with a strong understanding of reablement and empowering customers to live independently.
- A Level 3 diploma in Adult Care (or level 2 with relevant experience and willingness to study for level 3
- Great leadership skills with experience in mentoring or guiding teams in a care setting.
- A solid understanding of CQC standards risk management and safeguarding ensuring customer wellbeing and safety.
- The ability to build strong trusting relationships with customers families and professionals to deliver holistic person centred care.
- Working on a rota basis with shifts ranging between 7am to 10pm taking your turn to work days evenings weekends and bank holidays.You may from time to time be required to do some occasional nights during the initial set up of the services or in emergencies. We need you to be flexible to meet the needs of our customers but in return well give you the time you need to spend time doing the things that matter to you!
- Youll need an Enhanced DBS check done and we pay for that
- Confidence in using technology to manage care plans and records efficiently (dont worry we wont throw you in at the deep end)
