Job Summary:
Menorah Park located in Syracuse New York is seeking a Full Time LPN Care Coordinator to join our team. This individual will play a vital role in coordinating and managing the care of our residents ensuring they receive the highest quality of care. The LPN Care Coordinator will work closely with our healthcare team to develop personalized care plans and ensure that all medical needs are met. This is a fastpaced position that requires excellent communication and organizational skills.
Compensation & Benefits:
This is a fulltime hourly position with a pay rate of $28 per hour Monday thru Friday.
Employees will be paid on a weekly basis. Menorah Park offers a comprehensive benefits package including medical dental and vision insurance as well as paid time off and a retirement plan.
Responsibilities:
Requirements:
Active LPN license in the state of New York
Minimum of 1 year of experience in a longterm care or healthcare setting
Strong communication and interpersonal skills
Ability to work independently and efficiently in a fastpaced environment
Proficient in electronic medical records systems
Excellent organizational and timemanagement skills
Ability to prioritize and multitask effectively
Demonstrated empathy compassion and patience towards residents and their families
