PROGRAM: Integrated Case Management

JOB OVERVIEW:

The Community School General Case Manager (GCM) delivers longterm prevention case management services to youth (approximately 35 youth at a time) and their family who are experiencing barriers to regular school attendance overall academic achievement and general wellbeing. Through collaboration with key school personnel and community supports the GCM assists referred youth with case planning inschool interventions and ongoing inhome case management services. The GCM also engages other school youth periodically to participate in collaborative efforts to support the youth.

The GCM will prioritize purposeful collaboration with the Community School Coordinator school attendance team (including the partner attendance case manager) counselors and other support staff to efficiently and effectively promote positive change for referred youth.

The GCM is responsible for assessing the needs and strengths of the referred youth to inform the development revision and monitoring of a plan aimed at meeting the goals of the referred youth and family. The GCM is responsible for ensuring barriers are removed or lessened whenever possible including accompanying youth/parent(s)/guardian(s) to appointments or assisting in the completion retrieval and/or submission of documentation.

The GCM is an empathic compassionate creative and nonjudgemental member of a team of case managers who will be crosstrained to perform GCM duties as well as Attendance Case Manager duties to strengthen seamless transition for families who are navigating through supports available at the assigned community school.

EDUCATION/EXPERIENCE REQUIREMENTS:

  • Bachelors degree in social work or a related field
  • One year of direct service experience with parents and children in a community setting

KNOWLEDGE SKILLS AND ABILITIES:

  • Valid drivers license proof of car insurance and daily access to vehicle for travel to home visits schools and court hearings

  • Knowledge of demographics and culture of community school (internal and surrounding community)
  • Expert understanding of School District of Philadelphia attendance policies and procedures
  • Strong understanding of the Department of Human Services Standards for community school services
  • Extensive knowledge of resources and supports available to youth and families in Philadelphia and surrounding school communities

ESSENTIAL JOB FUNCTIONS:

Major highlights of the position requirements are noted below. Additional responsibilities may be assigned as required.

  • Through purposeful compassionate nonjudgemental and thorough case management service delivery prevent referred youth from progressing (further) into the formal child welfare system.
  • Be responsible for the development implementation monitoring and evaluation of individual youth plans to increase school attendance and remove/lessen barriers impacting the overall wellbeing of the assigned youth.
  • Attend and actively participate in MTSS meetings (or similar process) to receive referrals engage with providers and provide support to the school community.
  • Receive and act upon newlyreferred youth in a timely fashion.
  • Assess strengths and needs of youth and families referred through direct contact with school staff family and other appropriate programs/individuals and make necessary referrals to address needs.
  • Assess socioeconomic barriers to youths success and provide an appropriate level of support including but not limited to making necessary referrals and/or accompanying youth/parent/guardian to medical or service provider appointments.
  • Identify financial barriers impacting the assigned youths success and/or wellbeing and make the appropriate referral(s) (including but not limited to accessing emergency funds).
  • Facilitate and monitor referral(s) made for identified youth and/or family member(s).
  • Engage with parent/guardian and assigned youth inhome monthly (several visits monthly) to address barriers to regular school attendance support families to remove or lessen socioeconomic barriers and monitor plan success.
  • Monitor assigned youths school attendance and academic success by maintaining the appropriate level of contact with school personnel (at least monthly).
  • Retrieve regular attendance and academic progress for referred youth (and family members when appropriate) (at least monthly).
  • Complete the necessary documentation and follow the process to access emergency funds for assigned youth.
  • Make referrals for appropriate case management service delivery level of referred youth.
  • Support the facilitation of program/school workshops family forums and/or attendance initiatives (with partnering attendance case manager and/or program supervisor).
  • Assist in the implementation and monitoring of evaluation procedures for the program.
  • Participate in evaluations by external agencies as needed.
  • Participate in the evaluation of intraagency performance and quality improvement efforts.
  • Adjust practices and service delivery as informed by program evaluation and success or seasonal needs (i.e. school summer break) with the support/direction of the supervisor director or the Office of Children and Families.
  • Maintain accurate clear and timely documentation.
  • Participate in team meetings and biweekly supervisory sessions.
  • Participate in a minimum of 40 hours of training related to position responsibilities and agency requirements annually.
  • Adhere to all Carson Valley Childrens Aid Office of Children and Families Department of Human Services and programmatic policies and procedures.
  • Timely submission of all required clearances and physicals (periodic)
  • Timely submission of timesheets and timeoff requests
  • Timely submission of expense reimbursement documentation

TRAINING REQUIREMENTS:

  • Comply with agency annual training requirements.
  • Comply with programspecific training requirements (including OCF/DHS required trainings).
  • Crosstraining for Attendance Case Manager duties
  • Must pass CPR and First Aid Training

WORK ENVIRONMENT AND PRIMARY WORK LOCATIONS:

  • Flexible 40hour week
  • Evening and/or weekend hours required weekly
  • Must be flexible to work on evening projects and some weekend activities periodically
  • Frequent visits to the homes and/or community location of the youth and families referred:
  • School facilities
  • Community programs/organizations
  • Before/afterschool program facilities
  • Doctors offices
  • Administrative offices
  • Worksite: Assigned Neighborhood Center school and community

PHYSICAL AND BEHAVIORAL CONSIDERATIONS:

  • High energy needed

  • Ability to maintain composure in highstress and/or crisis situations with youth and adults
  • Ability to maintain appropriate professional demeanor at all times
  • Ability to travel to various locations within one workday (including but not limited to: in/out of vehicle entering various buildings including School District of Philadelphia school and administrative buildings)
