DEPARTMENT SUMMARY: The Senior Services department supports and enriches the lives of lesbian gay bisexual and transgender (LGBT) people 50 years and older with an overarching goal of successful aging in place. In this effort the department provides a broad array of social educational and support services to LGBT seniors all of which are free or lowcost. We have an expanding population of LGBT homeless population and are growing as a department to include on site affordable housing for LGBT seniors.
JOB SUMMARY: Under the supervision of the Senior Manager Social Services as part of the Seniors Services team the Senior Care Specialist II provides increased support and intervention with highneeds clients. In addition to developing individual care plans and promoting housing stability for LGBT seniors 50 throughout Los Angeles County the position will be responsible for creating positive relationships with assigned caseload which includes unhoused older adults and those with high acuity needs. Service provision may be delivered at any of our locations including but not limited to: Anita May Rosenstein Campus Ariadne Getty Foundation Senior Housing Triangle Square Metropolitan Community Church Mi Centro Center South Trans Wellness Center and other community sites.
ESSENTIAL FUNCTIONS:
Maintain an active caseload and ensure all documentation is completed following departmental procedures.
Provide crisis intervention including deescalation when issues arise with clients (utilizing strengthsbased harm reduction and trauma informed models of care).
Connect participants to emergency housing mental health services and substance abuse resources removing barriers when necessary to increase success.
Obtain clinical supervision when necessary.
Conduct indepth intakes assessments and reassessments covering psychological social physical and environmental concerns which includes clients functional and cognitive capacity and limitations.
Develop and implement a comprehensive care plan including all social services needed for client to meet their immediate or urgent needs as well as longerterm needs including housing stabilization and retention.
Routinely follow up and monitor participants to ensure that the care plan continues to meet the clients needs.
Assist participants with linkage to social service resources for seniors including but not limited to MediCal/MediCare Cal Fresh affordable housing mental healthcare physical healthcare legal services Veteran Affairs benefits inhome supportive services and home delivered meals.
Identify and develop family and other informal support systems for participants.
Problemsolve concerns raised by participants to promote positive participant interactions increase housing and program retention and reduce grievances.
Address client grievances in a timely manner with a successful resolution liaise with department management property management when applicable and/or community service providers to address and help mitigate concerns.
Develop behavioral contracts with clients when appropriate following a progressive discipline process whenever possible.
Develop working relationships with internal and external providers to build resources to address client needs.
Develop maintain and disseminate to staff comprehensive resources and referrals relevant to participant needs.
Provide weekly group activities to clients including but not limited to educational workshops on topics relevant to older adult needs.
Provide education to participants on Emergency Preparedness and conduct emergency drills.
Develop plan and execute onsite support group process by developing outreach and marketing plans to ensure target population needs are met.
Assist in training new hires in the Senior Care Specialist I position including shadowing of home visits comprehensive assessments case documentation and dropin case management.
Provide individual support to staff following complex interactions and facilitate weekly debrief opportunities for staff around difficult client situations and sharing of resources.
Collaborate with housing navigation staff and property management staff to ensure efficient and wellcoordinated interagency collaboration to support successful housing outcomes.
Facilitate mediation when needed between community partners and the Center to ensure positive results that are client centered.
Collaborate with activities coordinators to provide suggestions and assist in the delivery of programs to meet client needs.
Attend relevant trainings related to development of resources for older adults.
Develop and execute annual staff development and retention training plan which includes regular staff feedback opportunities and training evaluations.
Participate in care management meetings allstaff meetings inservice trainings case consultations and other meetings as assigned to support care coordination.
Maintain complete and accurate documentation of service objectives and outcomes.
Other duties as assigned.
JOB QUALIFICATIONS AND EXPERIENCE:
Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.
A passion for the Centers work and its mission to make the world a better place for LGBTQ people.
Minimum 2 years of experience in senior care case management and communitybased service delivery.
Experience in related field from an accredited university or 4year degree in social work gerontology or psychology.
Experience maintaining confidential information understanding of Health Insurance Portability and Accountability Act (HIPAA) requirements and pass HIPAA certification training for HIPAA compliance (within 90 days).
Capacity to provide patient and positive customer service and support while being able to multitask and problem solve during a participant crisis.
Working knowledge of issues impacting LGBT seniors in ethnically and socioeconomically diverse urban and suburban environments.
Knowledge of social service resources for older adults including but not limited to MediCal/MediCare affordable housing mental healthcare physical healthcare legal services Veteran Affairs benefits and In Home Supportive Services.
Bilingual skills a plus (English/Russian English/Spanish American Sign Language preferred).
Demonstrated facilitation and interpersonal skills including being collaborative highly organized and working successfully within a team.
Knowledge of strengthsbased trauma informed models of service delivery.
Demonstrated skills in deescalation of participant behavior.
Demonstrated ability to manage multiple tasks under pressure in a fastpaced environment.
Ability to work alternative work hours during evenings and weekends when scheduled.
Excellent verbal written time management interpersonal research and computer skills (Microsoft Word and Excel).
Access to reliable transportation a current California driver license and the ability to be insured.
