DIRECT SUPPORT PROFESSIONAL (DSP) CHILDREN & ADULT GROUP HOMES
(Part Time: PM NOC & Weekends shifts)
In a warm loving group home setting 6 residents per home) DSPs provide 24/7 professional care to blind/visually impaired emotionally disturbed and/or multidisabled children and adults ages 559. As a DSP you will be responsible for providing compassionate care and assistance making a positive difference in the daily life of our residents. This is a nonexempt position that reports directly to the Group Home Administrator.
Hourly rate: $ 14.25/ hour
Job Duties Include:
Providing direct care to residents to increase daily living skills cognitive development socialization and behavior management
Provide supervision and support for our clients in meeting their goals as per each individual program
Assist clients with daily living needs and socialization skills such as meals preparation hygiene groom dress exercise and communication
Assist clients with prescribed medications
Develop a rapport with residents while building a caring and supportive relationship
Qualifications:
High School Diploma required
A minimum of 6 months of experience working with special needs medically fragile and/or developmentally disabled population preferably in residential settings
DSP 1 & DSP 2 certification preferred
Prior experience and compliance with Title 17 & Title 22 preferred CPI or ProAct certification preferred
RESIDENTIAL COUNSELOR SHORTTERM RESIDENTIAL THERAPEUTIC PROGRAM
(AM. PM & NOC shifts
The Milieu Counselor will act as advocates for our ShortTerm Residential Treatment Programs residents and their families. Additionally this position will ensure that the comprehensive Department of Children and Family Services (DCFS) case management needs of the residents are met and monitored.
Salary range: $ 15.61 $ 16.00 / hour
Job Duties Include:
Check the risk assessment each day to see what youth/s are on red alert check on the youth (in person or by phone) then document I in the Program Support Service (PSS) Daily Log.
Conduct a daily head count for the program as needed.
Create special incident reports (SIR) when: ProACT is used a youth makes suicidal statements injury to residents or staff absent without leave (AWOL) off campus out of eyesight property destruction or physical s
Create Internal Incident Reports when residents have gone AWOL off campus issues verbal threats etc.
Complete restraint/seclusion monitoring tool forms anytime: hands are placed on a youth (escort seated standing wall or floor) or for involuntary use of the Seclusion room; anytime a youth is placed in Seclusion; restraints need to be placed on a resident.
Complete a missing persons report when: a resident goes AWOL off campus and is gone for 1 hour. Involved in quarterly Team Decision Meetings (TDM) on each STRTP resident and in the regular review of goals established pertaining to these plans
Works closely with mental health clinicians and case managers to insure that needs and service plan goals and objectives are implemented monitored and revised as needed.
Participates in the audit process. Corrects audit findings within designated timelines as outlined by Quality Assurance and Training Coordinator.
Maintains confidentiality of all pertinent referral information to assure the individuals rights are protected.
Develops agreed upon transition plan and assesses continuing care needs in collaboration with the residential team parent(s) and placement agency
Ensures that all referral contacts followup and documentation meet regulatory compliance for all government and referring agencies (e.g. Title 22 HIPAA etc..
Qualifications:
Bachelors degree in a relevant field from an accredited college or university.
Knowledge of local regulations state laws and procedures.
Knowledge of social casework principles and methods.
Considerable knowledge of the Regional Center and Department of Children and Family Services systems and SSI and MediCal resources.
Minimum 1 year of experience making effective use of community resources establishing and maintaining an effective working relationship with children families staff and outside agencies.
Previous experience working in a residential setting preferred.
MILIEU COUNSELOR TRANSITIONAL SHELTER CARE PROGRAM (AM PM & NOC shifts
The Transitional Shelter Care (TSC) provides a safe and therapeutic environment to children ages 2 days 17 years with a focus on 05 years of age who have been removed from their homes due to abuse neglect or abandonment and need immediate placement. The primary goal is to provide the TSC residents therapeutic care specialized treatment recreational activities and crisis intervention.
Salary range: $ 15.61 $ 16.00 / hour
Job Duties Include:
Ensures the health safety and welfare of our residents in the program.
Assists in planning organizing and supervising milieu programs such as recreational and therapeutic activities which promote developmental competencies and skills targeted to childs unique and special needs.
Provides supportive guidance working closely with mental health clinicians supervisors and social workers.
Reports any incidents/accidents according to incident reporting procedures and protocols within designated timelines as outlined in the reporting procedures
. Provides transportation to residents to medical appointments community outings meetings or other locations by driving agency vehicles.
Ensures a safe and clean living environment participating in household cleaning and maintenance activities.
Qualifications:
Bachelors Degree from an accredited college or university in Child Development Special Education Psychology or related field required.
Minimum 1 years residential or childcare experience preferably emergency shelter.
Current ProAct Certification preferred.

We are seeking a Medical Secretary who is responsible in ensuring Wayfinder is in compliance with licensing and contractual requirements for the timely completion of dental exams initial and annual physicals completes forms and arranges transportation to urgent care and emergency room visits under the direction of nursing services staff.
This is a nonexempt position that reports directly to the Director of Nursing Services.
ESSENTIAL FUNCTIONS and Responsibilities:
Creates new charts upon client admission.
Acts as a liaison to gather important information such as immunization history insurance numbers names of any specialists etc.
Verifies clients insurance status and contacts health insurance providers by phone and fax to ensure proper reimbursement for Wayfinder.
Keeps charts updated and maintains copies of medical alerts and copies of insurance information for scheduled and emergency appointments.
Keeps information up to date and monitors due dates for appointments and services.
Calls and schedules Medical Dental Specialist and Procedures (CTs MRIs Ultrasounds etc..
Tracks all medical dental specialty medical appointment and psychotropic authorization data into the Health Services Database accurately and in a timely manner.
Completes all related documentation for medical dental laboratory and specialty medical appointments on appropriate forms and requisitions.
Ensures that psychotropic medication authorization renewal forms are provided to the psychiatrist in a timely manner and may assist nurses by faxing completed forms to the court(s).
Assists the nurse(s) in coordinating the schedule of the consultant psychiatrists. This includes providing charts according to the psychiatrists appointment schedules.
Assists with nursing services emails and requests from a wide variety of sources such as mental health probation DCFS public health nurses school nurses IEPs cottage staff and quality assurance.
Assists in billing input for medical department DMH services.
Qualifications:
High School Diploma or equivalent and special medical or office training.
Minimum of one 1 year experience in a medical office.
Medical Assistant or Administrative Assistant Certification preferred.
Advanced computer skills in Microsoft Office Suites highly preferred.
RESIDENTIAL LICENSED VOCATIONAL NURSE (LVN) (NOC Parttime & OnCall shifts)

Education School who will be responsible for implementing an individualized comprehensive program for our students who are multidisabled and blind or visually impaired.
This is an exempt position that reports directly to the Principal of the Special Education School.
Major Duties and Responsibilities:
You will implement an individualized comprehensive program based on the Critical Skills Model while focusing on the students development of compensatory skills and selfesteem.
You will participate in educational assessment by using selected tools involving parents designated instructional service staff and other
relevant individuals.
You will develop students Individualized Educational Programs (IEP) with the assessment team based on assessment findings and priorities
identified by parents.
You will be responsible for providing instructional activities which are ageappropriate functional motivating and which reflect IEP goals.
You will provide diverse learning opportunities such as communitybased instruction daily living skills arts and crafts music and
movement vocational social integration swimming and recreational activities.
You will provide handson supervision of staff volunteers and interns in the Special Education School to ensure quality teaching and
application of the curriculum is carried out through careful monitoring of individualized programming as per every students IEP.
Qualifications:
Bachelors degree required; Masters preferred.
Must have a California Specialist Credential in one of the following: Visual Impairment Moderate to Severe Disabilities Severely
