Case Manager FullTime
*Fulltime (MF 8am4:30pm)*
Experience in physical medicine and rehabilitation strongly preferred.
SSM Health Day Institute is seeking a Case Manager to work in our specialized outpatient rehabilitation program and have the ability to float within the region to assist other Day Institute facilities. We have ten centers across the St. Louis Metro area serving individuals recovering from a traumatic injury or illness who no longer require 24hour nursing or acute rehabilitative care. We take a comprehensive multidisciplinary approach to build skills and maximize independence. Responsibilities
Under general direction monitors and assesses patient plan of care from preadmission to post discharge.
As part of the treatment team assists patient/family in goal setting to maximize patients independence in both home and community. Interacts with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program.
Qualifications
Professional Healthcare Degree in either
- Nursing Social Work
- Licensed PT OT SLP
One year medical rehab case management experience (preferred)
