Role:Case Manager C3 Skid Row
Reports to: Program Manager

Status: Full Time Non Exempt (hourly) NonManagement
SUMMARY
With the support of and in partnership with the C3 team the Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals and is responsible for identifying engaging and providing support to vulnerable homeless program participants including providing aftercare and follow up support once housed. The C3 Case Manager will engage homeless individuals living on the street in Skid Row to provide linkages to housing and supportive services. These teams are part of a largescale streetbased engagement effort in the Metro Los Angeles Area (Service Area 4. The goal of these teams is to create a countywide network of multidisciplinary integrated streetbased teams to identify engage and connect or reconnect homeless individuals to interim and/or permanent housing and supportive services. The Case Manager provides information and support for program members and assists members in linking to a variety of resources and services as needed. The Case Manager will model competence in wellness recovery and coping and is responsible for maintaining the highest quality services and efficiency of operations. The C3 Case Manager will provide intensive case management services to a caseload of highly vulnerable individuals living on the streets of Skid Row.
