GENERAL SUMMARY/ OVERVIEW STATEMENT: Under the general supervision of the Director Perfusion Services operates extracorporeal circulation equipment during medical procedures requiring support or temporary replacement of patients circulatory or respiratory functions. Prepares and administers blood products and prescription solutions through bypass circuitry. PRINCIPAL DUTIES AND RESPONSIBILITIES: Clinical Functions Performs duties to support cardiopulmonary bypass; maintains and demonstrates competency in the performance of all responsibilities. Regularly participates in the on call schedule Appropriate choice and use of disposable and heartlung machine equipment and safety devices for the performance of CPB. Appropriate maintenance according to our practice guidelines of the patients physiological parameters/variables during CPB. Prepares for surgery by assembling extracorporeal circuit including connecting tubing oxygenators pumps and filters. Primes and debubbles circuit. Prepares administers and monitors flow of prescribed solutions and medications including circuit prime cardioplegia and blood products used during procedures requiring extracorporeal support. Develops perfusion care plan and calculates appropriate perfusion formulae to be communicated during preoperative briefing. Manages extracorporeal circuits to achieve conditions ordered by surgeon including CPB deep hypothermic circulatory arrest (DHCA) coronary perfusion venovenous bypass or partial bypass. Maintains supply of blood in system and makes any adjustments needed to achieve conditions specified by surgeon or anesthesiologist such as changes in blood temperature oxygen/carbon dioxide ratio in blood or patients electrolytes. Receives and interprets blood gas values and assesses adequacy of perfusion. Adjusts and monitors heartlung machine settings according to clinical practice guidelines for CPB. Monitors and maintains anticoagulation according to clinical practice guidelines for CPB. Monitors interprets adjusts and calibrates various detection devices electromagnetic flow meters heat exchanges devices pulsatile devices blood processing equipment and bypass circuitry used in cardiothoracic and vascular procedures. Maintains a working knowledge of appropriate equipment and strategies utilized for complex cardiothoracic and vascular surgery and liver transplantation. Disassembles extracorporeal circuits and mechanical components of the heart/lung machine and cleans all nondisposable items and disposes of contaminated materials. Records updates and maintains permanent records of all cardiovascular bypass procedures. Participates in emergency operation of intraaortic balloon pump during cardiac procedures. Prepares and manages extracorporeal support for Surgical Oncology procedures including isolated limb perfusion and hyperthermic intraperiotoneal chemoperfusion (HIPEC). Prepares and manages extracorporeal support for complex vascular surgery including descending aortic aneurysm repair. Prepares and manages blood conservation systems. Prepares and manages paracorporeal ventricular assist devices for patients requiring temporary ventricular support. Prepares and manages extracorporeal membrane oxygenation circuits for venoarterial and venovenous support. Maintains optimum quality assurance in all aspects of patient care procedures. Operational FunctionsParticipates in departmental projects and processes including but not limited to: Quality improvement Continuing education Quality control Scheduling Student education and development Inventory management QUALIFICATIONS: Requires: B.A. degree with science courses: biology physiology chemistry and physics.Completion of a Perfusion Education Program accredited by the Commission on Accreditation of Allied Health Education Programs and eligibility to complete certification with the American Board of Cardiovascular Perfusion (ABCP).ABCP Certification with successful completion of Perfusion Basic Science and Clinical Applications in Perfusion exams. ABCP recertification must be renewed annually. New graduates must be certified within one exam cycle of eligibilityLicensed by the Commonwealth of Massachusetts Division of Professional Licensure of Perfusionists. SKILLS/ ABILITIES/ COMPETENCIES REQUIRED: Knowledge of membrane oxygenators complex adult cardiac surgery centrifugal pumps roller pumps isolated limb perfusion DHCA intra aortic balloon pump heart lung and liver transplantation paracorporeal ventricular assist devices and various myocardial protection techniques such as cold crystalloid and blood warm continuous blood cold fibrillation/arrest and blood gas interpretation.Must possess current BLS certificateEffectively operate monitor and troubleshoot equipment and processes.Ability to develop and implement effective solutions including during critical stressful situations.Must be able to work independently and as part of a team. Ability to effectively build and manage interpersonal relationships.Demonstrate comfort and facility in working within an operating room setting. Use proper sterile technique and appropriate PPE. Work safely and ensure the safety of patients and others.Is knowledgeable and compliant in all hospital State and Federal regulatory requirements including hospital policy and procedures (where applicable to the performance of the job) Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and Health Insurance Portability and Accountability Act of 1996 (HIPAA). WORKING CONDITIONS: Well ventilated well lit operating rooms and assembly rooms. Hazards present from exposure to blood combustible anesthetic or disinfecting agents are minimized when safety precautions are strictly adhered. Long periods of time standing and walking. May push pull and carry supplies and equipment. Must be able to lift up to 40 pounds. Must be available for on call coverage. SUPERVISORY RESPONSIBILITY: No responsibilities for the work of others.
